BEN BIKMAN 4 | CALORIES on metabolism: high carb: slows low carb: speeds net: 300 calories/day
http://www.DoctorsToTrust.com
presents episode 1011 | Dr Ben Bikman
TheKetoNutritionist podcast
Sub-clinical inflammation...
-starts in fat cells
2 ways to get fat
1-hyperplasia: cells multiply, stay small
[women, blacks, caucasians mostly]
2-hypertrophy: cells get larger in size
[most people, men...]
"fat cell can grow 20x in size;
pushed further from capillaries;
harder to get oxygen;
releases pro-inflammatory proteins
increases blood flow
So, suffocating fat cells are belching
out pro-inflammatory cytokines
[C-Reactive Protein is one]
-trying to increase blood flow
THIS INCREASES INFLAMMATION
THROUGHOUT BODY
When get to muscle cell:
-directly cause insulin resistance [IR]
INFLAMMATION is 1 direct cause of IR
Fat tissue is an aggressively active
endocrine organ...
-secreting many protein & hormones
into blood all the time
David Ludwig, Harvard, papers:
-give people 2 isocaloric meals
-exact same calories
-monitor their calorie expenditure
high carb: SLOWED METABOLIC RATE
low carb: HIGHER METABOLIC RATE
Difference? 300 calories a day

presents episode 1010 | Dr Ben Bikman
TheKetoNutritionist podcast
Massive study from UCLA...
-plotted every heart attack patient's
LDL: result: bell curve
-peak of curve was normal
"More people had heart attacks with
good LDL levels"
Size and density of LDL MATTERS
-small, dense LDL atherogenic
[blood vessels develop lesions]
-large, buoyant healthy
Higher triglyceride/HDL=small, dense LDL
Should be triglyceride/HDL ratio to
determine statin use...
If high, then statins may be useful, but
-rather: simply adjust diet to lower
triglycerides in blood!
-can be done in a few weeks
-will reduce size & problematic LDL
-INCREASE HEALTHY LDL
IN women
-lower LDL=higher Alzheimers, blood
based cancers; serious infections UP
15 times the risk!

USA not in top 20 countries in diabetes...
-global problem
INSULIN RESISTANCE: 2 parts
1-some cells NOT RESPONDING to insulin
2-HYPERINSULINEMIA too much insulin
so healthy cells are overwhelmed!
High blood insulin: okay for
-insulin resistant cells:
extra insulin gets in
BUT for normal cells: extra insulin makes
them RESPOND TOO MUCH
-that is a problem
Metabolic health? Flexibility:
-eat starch? go to glucose burning
-fast 12 hours? go to fat burning
[insulin dictates which fuel is used]
-insulin high: glucose burning
-low: fat burning
If metabolically UNHEALTHY:
-insulin is always elevated, so
-stuck in glucose burning mode
Diseases of insulin resistance...
Skin
-acanthosis nigricans
[hyperinsulinemia driving melanin]
Brain
-insulin resistance driving Alzheimer's
Liver
-insulin resistance driving fatty liver
INFERTILITY
-ED in men
-PCOS in women
INFERTILITY
PCOS: due to high insulin
-ovary cells STILL SENSITIVE to insulin

http://www.DoctorsToTrust.com
presents episode 1005 | Dr Nadir Ali
Dr Nadir Ali podcast
During fasting, liver charged with providing fuel...
-every part of body joins in the fast,
-except the brain
-liver takes fatty acids from fat cells,
converts them into ketones [for brain]
-takes glycerol from triglycerides & aminos from muscle
-thru gluconeogenesis makes glucose [from fat & protein]
-also for brain [needs some glucose]
Breaking the fast with a sweet potato...
-when liver is insulin senstive,
-takes in glucose that is not immediately needed by muscle
WHEN EATING, liver shuts down hepatic glucose output
If eating standard American diet: glucose OVERLOAD
-so need a whole lot of insulin to bring glucose in
If normal [not insulin resistant] will:
- convert some glucose into glycogen
-shut down glucose output
-converts rest into fat [=carbs signal fat production]

Carbs turns into blood
glucose; into muscle

NADIR ALI b CLASSIC | LDL is poor health marker QUALITY of LDL is key To metabolic health
http://www.DoctorsToTrust.com
presents episode 999| Dr Nadir Ali
Dr Nadir Ali podcast
for example: a cardiologist feels good when he can help someone lower their cholesterol
"this is fantastic we have improved your health" they are looking at the wrong marker.
For me: is this guy feeling better because: blood sugar is lower?
Joints feel better? Blood pressure lower?
Spending less on medication's?
these are better surrogate markers than a blood marker.
Cardiologist have been focusing on cholesterol,
which is the wrong thing cholesterol is a surrogate marker for insulin resistance.
We should focus on the quality of the cholesterol,
which is an indication of metabolic health.
A typical consultation is: "your stress and ultrasound tests are good,
let's go up on cholesterol medicine let's increase your blood pressure medicine,
go for a low fat diet and exercise. See you in six months.

JOHN CAMPBELL b | Doctor-patient relationship DESTROYED by CENTRAL PLANNING
http://www.DoctorsToTrust.com
presents episode 998 | Dr John Campbell
Dr John Campbell podcast
Dr Robert Clancy,
"I have acted upon 2 principles
1-Integrity of doctor-patient relationship
Lots of discussion with patients &
collectively make a decision
"This is completely out the window now"
We are told, both patient & doctor:
'this is how it is going to be'
Had to tell them: stay home until cannot
breathe...then come in
"First time in 50 years this has happened"
Doctor-patient relationship was distorted
by misinformation in the media...
-we weren't being told the truth
-yet, the truth was there...
-we knew vaccines were not going to
work very well
-knew about the side effects at end 2020
Now, moving decision-making process
from country-level to WHO:
doctor-patient relationship may never
be repaired
Principle #2: science-based medicine...
-not just randomized, control trials
-but, each patient different than the
average in a study or textbook
Every patient has different background;
different conditioning factors; different
life strategies
-need to look at the process of disease
-especially rare immunological ones
I work with
-must work with patient on what approach
makes logical sense

JOHN CAMPBELL a | IMMUNE RESPONSE to mRNA spike protein may ATTACK HEART MUSCLE
http://www.DoctorsToTrust.com
presents episode 997 | Dr John Campbell
Dr John Campbell podcast
These lipid nano-particles carrying the
RNA: injected into arm,
-distributed throughout body
-blood vessels in myocardium &
coronary arteries & capillaries &
venules
Circulate through heart & will join in
with endotheliel cells
-takes over cell's protein-producing
apparatus
-causes cells to excrete foreign,
synthesized spike protein to surface
-causing inflammatory immune response
-cytotoxic T cells destroy the cells,
causing inflammation in the heart
This process applies everywhere...
Why worry so much about the
myocarditis & pericarditis side effects?
1-if heart stops working, it is obvious
2-can measure triponins
[easily tested cardiac marker]
In other organs it is harder to see
Dr Gad Saad asks Dr Paul Offit:
Is there problematic link between mRNA
vaccine and heart inflammation?
-Yes. Link with myocarditis & pericarditis
-don't know why
-may be that the spike proteim mimics one
of the proteins on heart muscle cells
-heavy chain of actin
Therefore, when
-making immune response to spike protein,
also
-making it to heart muscle

JEFF VOLEK b | EXERCISE to reduce calories BACKFIRES: Resting energy declines!

Metabolic Link podcast w/ DOM D’AGNOSTINO
Couple of decades ago, metabolic ward,
twin [monozygotic] studies on weight loss...
-exercise effects
-cycle 600kcal/day
-fed calories that would have kept their
weight stable if not cycling
-7 sets of twins
-1 of each pair did the cycling
Results: wildly differing weight changes
If you say that 3500 kcal = 1 pound fat loss...
-expected: over several weeks of the study
=several kg weight loss in cyclists
1 pair: BOTH lost this much weight
1 pair: BOTH lost no weight
Within twin concordance was much
higher than across twin pairs
Example: if you lost 1 kg, so did your twin
ABILITY OF EXERCISE TO CONTRIBUTE
TO WEIGHT LOSS IS DEPENDENT ON GENETICS!!
Dom:
"Exercise caused metabolic adaptations
that enhanced metabolic efficiency"
-which preserved energy through
reduced T3
Jeff
-was not a popular result in training circles
"What accounted for MOST OF FAT LOSS
was resting energy expenditure"
-it DECREASED FOR EXERCISING TWIN
-would think the opposite, but
'In context of exercising, resting energy use goes DOWN'

-want it to be enjoyable to eat so
catering to their preferences if we can
-must be in ketosis
-customize carb & protein intake
-provide education
-adjust if not in ketosis

PAUL MASON k | In vulnerable folks: DAIRY & GLUTEN can trigger autoimmune
http://www.DoctorsToTrust.com
presents episode 994 | Dr Paul Mason
Nutrition with Judy podcast
Published study of 2 African tribes in 1920s:
-Masai: carnivore [meat, blood, dairy]
-Akakuyu: vegetarian
Both tribes genetically similar
Results of observation and measurements:
Masai males vs Akakuyu males:
-5 inches taller
-23 pounds heavier
-50% stronger
"Bony deformatties, dental caries, anemia, pulmonary conditions
much more prevalent among the Akakuyu...
-intestinal statis [constipation] & rheumatoid arthritis
more common in Masai
So: carnivore, including dairy, makes one
stronger & more robust than vegetarians
Constipation & arthritis [autoimmune]:
-casomorphine [opiod peptide in dairy]
-morphine [opiate] cause constipation
Not well known, but my patients find that:
-in vulnerable people:
-remove dairy [casein] & gluten from diet, constipation relieved
-also an autoimmune trigger:
-can cause rheumatoid arthritis
Improved when dairy & gluten removed
Masai were drinking raw, A2 milk!

PAUL MASON j | CHRONIC INFLAMMATORY DISEASES? Low T? carnivore can fix both
http://www.DoctorsToTrust.com
presents episode 993 | Dr Paul Mason
Nutrition with Judy podcast
Q: sex-hormone binding globulin [SHBG]: will eating only meat,
cause it to go up & testosterone to drop?
A: SHBG is high enough in carnivore dieters.
If it goes higher, we can easily monitor it
-old thinking: SHBG would bind to
testosterone & prevent it being active
-in fact: when bound, it still have
physiologically active
-the old thinking is NOT TRUE
Lean mass hyper-responder phenotype [Dave Feldman],
-on carnivore: LDL UP & triglycerides DOWN & HDL UP
-SHBG can be added to this phenotype
these almost always have LDL up & SHBG up
-multiple things can make SHBG go up;
but along with higher HDL & LDL;
lower triglycerides...
"This does not impair athletic performance
at all"
People, with chronic inflammatory conditions, will go onto
carnivore eating protocol-which are associated with low T...
-when inflammatory conditions are relieved by
carnivore eating, then
-testosterone returns to normal
Testosterone has diurnal variation so levels
vary throughout the day

JAY VINCENT | LEG PRESS safer than squat & SAME EFFECT for building STRENGTH
http://www.DoctorsToTrust.com
presents episode 992 | Jay Vincent
Jay Vincent podcast
"I train legs once a week with a
leg press or squat at: 275 - 315#
-one set to failure
-followed by leg curl
-one set to failure
-then knee or leg extension
-one set to failure
-heel raise, to failure
Total: 4 exercises, once a week
Strength does transfer...
[from super slow lifts to normal]
-the more you practice; the more
load you can move [skill + practice]
"Currently, practicing squats with
heavier loads;
with more practice with heavy
loads, will be able to get lower"
Body must develop the muscle
recruitment & skill to go LOW
"I am allowing time to adapt &
practice going so low, heavy"
Used a wedge under feet to give
more ankle extension support...
What is better:
SQUAT or LEG PRESS?
-I alternate between squat & press
-both equally effective:
IN DEVELOPING STRENGTH
"Squat is better at improving your
ability to move weight in squat"
-if only do leg press, skill in squat
will suffer due to lack of transfer
But, for strength SAME
"I like squat because I am good
at it. Like others like bench press
because they excel at it"
For most people, stick with
leg press, so you can
FOCUS ON FATIGUING THE
TARGET MUSCLE AREAS
-hips
-thighs
-lower torso
-in a MORE SAFE EXERCISE
than the squat

PAUL MASON i | High-carb ATHLETES OUTPERFORMED BY keto-adapted athletes
http://www.DoctorsToTrust.com
presents episode 991 | Dr Paul Mason
Nutrition with Judy podcast
2018 McSwiney study of high level athletes for 12 weeks...
test 1- peak power output
test 2 - critical, sustained power output [anaerobic]
Results:
test 1: keto-adapted: better time by 3 min.
& higher peak power output
test 2: keto-adapted: OUTPERFORMED
those on high carb diets
"Clear evidence that keto-adapted dieting
can provide improved athletic performance"
Q - Do endurance athletes need carbs? A - NO
Q: Why do some athletes or weightlifters feel 'better' when having
some carbs before a workout?
A: research, 15 years ago:
Rating of Perceived Exertion
-release of dopamine may make people
feel that exercise is easier
But, in actual testing: no difference
Judy Cho [w/ Mano Henselmans]
"It is the placebo effect
-It takes AT LEAST 4 MONTHS to get KETO-ADAPTED
How do we know?
-when body burns fat, initially turns into ketones
-if body mechanism is not fully upregulated,
they will not convert, but be expelled
-uric acid & ketones compete for excretion
from kidneys into urine, SO
-if elevated KETONES, will prevent some
URIC ACID from leaving body, SO
-uric acid level will INCREASE IN BLOOD
"We can check uric acid blood level to see
if you are keto-adapted & monitor the length of time"
-Takes at least 12 weeks
Question?
-Sometimes, people having trouble with GOUT,
beginning a keto/carnivore eating approach , will still
experience gout attacks [in a diminishing progression]
-Could this be the reason?
A-complicated, but could indeed be...

PAUL MASON h | Study ULTRAMARATHON RUNNERS: fat-adapted = SAME GLYCOGEN AFTER FULL MARATHON RACE
http://www.DoctorsToTrust.com
presents episode 990 | Dr Paul Mason
Nutrition with Judy podcast
Question: are carbs needed for athletic performance?
-1960s technique to measure muscle glycogen
-high performance deteriorated when
glycogen was depleted
-glycogen made from glucose...
-carbs make glucose, so
KINDERGARTEN SCIENCE:
-high anaerobic, power, strength
performance will improve
with CARBS
"All resulting from one test capability"
No one questioned: are carbs NEEDED for glycogen?
-NOT THE CASE
-single 'evidence' that glucose is needed for glycogen stores
2016: Dr Jeff Volek study:
Ultramarathon runners: 2 groups
1-keto-adapted runners
2-high-carb adapted runners
Prior to study measured muscle glycogen
-SAME IN BOTH GROUPS
Result: after running marathon: NO DIFFERENCE IN GLYCOGEN
AFTER: nutrient replenishment--NO DIFFERENCE IN GLYCOGEN
-Proof: keto-adapted athletes maintain glycogen stores
