Glucose is the main source of energy production in our body. Normal blood glucose levels should be between 60 to 140 mg/dl in order to supply cells of the body with its required energy. Euglycemia, or blood glucose within the normal range, is naturally ideal for the body’s proper functioning. A good balance between hormones of the pancreas, including insulin and glucagon, intestinal and adrenal hormones is required to maintain normal blood glucose levels.
Cells can also use proteins and fats in order to produce energy. However, glucose is the major energy source in our body.
Glucose is absorbed through the intestines in to the blood stream and then stored by the body in the form of glycogen within the liver and muscle tissue.
Regulation of blood glucose is largely done through the pancreatic hormones. Insulin, and glucagon are the two most important ones. Each of these hormones have opposite actions.
Insulin, secreted by the pancreatic beta cells, lowers the blood glucose levels. Whereas glucagon, secreted by the pancreatic alpha cells, increases the blood glucose levels.
Insulin decreases blood glucose levels by the following mechanisms.
Insulin promotes glucose uptake by the cells, especially in the muscle and fat tissue.
It also reduces the production of new glucose by the liver. In addition, insulin promotes glycogen production in the liver, using glucose. Through these mechanisms, insulin reduces the blood glucose level to a normal after a meal.
On the other hand, glucagon has the opposite of these actions. And its main job is to maintain a healthy blood glucose level between meals.
Hypoglycemia, also known as low blood sugar, is a clinical situation characterized by a reduction in plasma glucose concentration to a level that is far below the normal.
The glucose level at which an individual becomes symptomatic is highly variable, although a plasma glucose level less than 50 mg/dl is generally considered the threshold.
Causes of hypoglycemia are varied, but it is seen most often in diabetic patients. Hypoglycemia may result from medication changes or overdoses, infection, diet changes, metabolic changes over time, or activity changes. Other causes include gastro-intestinal problems, fasting, insulin secreting tumors, endocrine problems, and liver disease.
Some common medications that can cause hypoglycemia include, oral hypoglycemics such as phenylbutazone, insulin, propranolol, clofibrate, sulfonamides, isoniazid, methanol, methotrexate, tricyclic antidepressants, thiazide diuretics etc.
Early signs of hypoglycemia may include the following.
Heavy sweating. Dizziness. Extreme hunger. Fatigue. And a fast and pounding heartbeat, also known as palpitations.
As hypoglycemia worsens, signs and symptoms may include the following.
Confusion. Abnormal behavior. Visual disturbances such as blurred vision. Seizures. And loss of consciousness.
Medications that are used in the treatment of hypoglycemia include the following.
Glucose supplements such as dextrose.
Glucose elevating agents such as glucagon. And insulin secretion inhibitors such as diazoxide and octreotide.

Contraception, also known as birth control, is the deliberate use of artificial methods or other techniques to prevent pregnancy as a consequence of sexual intercourse.
A women can get pregnant if a man's sperm reaches one of her eggs, also known as ova, that is produced by the ovaries.
Contraception tries to stop this from happening by keeping the eggs and sperms apart, preventing egg production by the ovaries, or by preventing the combined egg and sperm, which is known as the fertilized egg, from attaching to the mother's womb, a process known as implantation.
According to their mechanism of action, female contraceptives can be categorized in to 5 main types.
These include;
Mechanical barriers.
Hormonal contraceptives.
Intra Uterine Devices, commonly known as IUDs.
Female sterilization.
And emergency contraception.
Let's talk about these methods in more detail.
Mechanical barriers include female condom, diaphragm, and the cervical cap. Female condom prevents pregnancy by acting as a barrier to the passage of semen in to the vagina. However, it is important to note that simultaneous use of both femaleand male condom is not recommended because they may adhere to each other, leading to displacement of either device.
Diaphragm prevents pregnancy by acting as a barrier to the passage of semen in to the cervix.
Once in position, the diaphragm provides effective contraception for 6 hours.
Cervical cap acts as a mechanical barrier for sperm migration in to the cervical canal.
Hormonal contraceptives prevent pregnancy through various mechanisms. Hormonal implants, Contraceptive injection, Oral contraceptive pills, and Contraceptive patches belong to this group.
Hormonal implants, commercially known as Nexplanon implant, consists of one nonbiodegradable rod of 40% ethylene vinyl acetate and 60% etonogestrel. It prevents pregnancy by suppression of ovulation, thickening of the cervical mucus, which in turn prevents the penetration of sperm, and by prevention of endometrial growth and development.
Contraceptive injections are composed of the hormone progestin.
Depo-Provera, is a well-known brand name for contraceptive injections. These injections work by inhibition of ovulation, thickening the cervical mucosa, and by preventing implantation.
Oral contraceptive pills contain artificial versions of the female hormones estrogen and progesterone. The main mechanism of action of these pills is to suppress ovulation.
Contraceptive patches release estrogen and progesterone directly in to the skin and then in to the blood stream, and prevent ovulation.
An IUD is a small, T-shaped plastic and copper device that is put in to your uterus by a healthcare professional.
It protects you from getting pregnant for 5 to 10 years.

Female sterilization is a permanent surgical option to prevent pregnancy.
During this surgery, the fallopian tubes are blocked or sealed to prevent the eggs reaching the sperm and becoming fertilized.

Type II Diabetes _- Causes, Risk Factors And Treatment
Glucose is the main source of energy production in our body. Normal blood glucose levels should be between 60 to 140 mg/dl in order to supply cells of the body with its required energy. Euglycemia, or blood glucose within the normal range, is naturally ideal for the body’s proper functioning. A good balance between hormones of the pancreas, including insulin and glucagon, intestinal and adrenal hormones is required to maintain normal blood glucose levels.
Cells can also use proteins and fats in order to produce energy. However, glucose is the major energy source in our body.
Glucose is absorbed through the intestines in to the blood stream and then stored by the body in the form of glycogen within the liver and muscle tissue.
Regulation of blood glucose is largely done through the pancreatic hormones. Insulin, and glucagon are the two most important ones. Each of these hormones have opposite actions.
Insulin, secreted by the pancreatic beta cells, lowers the blood glucose levels. Whereas glucagon, secreted by the pancreatic alpha cells, increases the blood glucose levels.
Diabetes is a condition characterized by elevated blood glucose levels than normal.
There are 2 main types of diabetes. Type 1 diabetes usually starts before the age of 20. It is an autoimmune condition, characterized by the destruction of the pancreatic beta cells, leading to absolute insulin deficiency. So, these people will have to take exogenous insulin throughout their lives.
On the other hand, type 2 diabetes, which is the most common type, usually appears in adults who are obese.
Type 2 diabetes is characterized by a combination of peripheral insulin resistance and inadequate insulin secretion by pancreatic beta cells.
In insulin resistance, the cells do not respond to insulin as in normal individuals. So, their glucose uptake will be impaired. And as a result, glucose will build up in the blood, leading to high blood glucose levels.
For type 2 diabetes to occur, both insulin resistance and inadequate insulin secretion must exist. For example, all overweight individuals have insulin resistance, but diabetes develops only in those who cannot increase insulin secretion sufficiently to compensate for their insulin resistance.
The etiology of type 2 diabetes appears to involve complex interactions between environmental and genetic factors.
Major risk factors for the development of type 2 diabetes include the following.
Age greater than 45 years.
Sedentary lifestyle.
High calorie intake.
Obesity.
Family history of type 2 diabetes in a first-degree relative, like parents or a sibling.
Hispanic, Native American, African American, Asian American, or Pacific Islander descent.
High blood pressure.
High blood cholesterol.
History of gestational diabetes in women.
And polycystic ovarian syndrome.
Many patients with type 2 diabetes are asymptomatic, and their disease remains undiagnosed for many years.
Classic symptoms of diabetes include, polyuria, or increased urination, polydipsia, or increased thirst, polyphagia, or increased hunger, and weight loss. Other symptoms that may suggest hyperglycemia include blurred vision, lower limb paresthesia, lack of energy, poor wound healing, and frequent infections.
Long term diabetes is associated with the development of a number of complications.
These complications can be broadly divided in to 2 groups; microvascular complications, and macrovascular complications.
Microvascular complications typically involve the arteries. 3 major microvascular complications are, diabetic retinopathy, which can lead to adult onset blindness, diabetic nephropathy, which is the leading cause for chronic kidney disease, and diabetic neuropathy, which affects the peripheral nerves.
Macrovascular complications increase the risk of heart attack, stroke, and peripheral vascular disease.
Lifestyle changes are the first line treatment of type 2 diabetes.
These include, regular exercising, low calorie diet, quitting smoking, weight loss etc.
If these changes are unable to provide a considerable improvement, then anti diabetic drugs are indicated.
Because type 2 diabetic patients are not completely lack of insulin, initially they do not require exogenous insulin.
Metformin is the first line drug in the medical therapy of type 2 diabetes. In addition, other anti diabetic drugs can also be used in combination with metformin.
However, as the disease progresses, the pancreas will gradually lose its ability to secrete insulin, and ultimately, these patients will also require exogenous insulin to maintain a healthy blood glucose level.

Kidney stone disease, also known as nephrolithiasis, is a common urological problem among many people.
Kidney stones are often medically referred to as renal calculi. These calculi typically originate within the kidneys.
However, sometimes they can pass down in to the ureters, and bladder, and can cause obstruction of the passage of urine.
Four major types of renal calculi present.
Calcium stones, accounting for about 75% of cases.
Magnesium ammonium phosphate stones.
Uric acid stones.
And cysteine stones.
In addition, certain medications also responsible for kidney stones.
Some of these include indinavir, Atazanavir, Silicate, and Sulfa drugs such as sulfasalazine, and sulfadiazine.
Hypercalciuria, or high calcium in urine is the most common metabolic abnormality that leads to the formation of calcium stones.
Some cases of hypercalciuria are related to increased intestinal absorption of calcium some are related to hormone abnormalities, like hyperparathyroidism.
In addition to hypercalciuria, other factors that leads to increased formation of kidney stones include low fluid intake.
Family history of kidney stone disease.
Hyperuricemia, or high uric acid levels in blood due to high intake of protein-rich foods, or due to inherited conditions.
Urinary tract infections.
And low citrate levels in blood.
Well, what are the most common symptoms of kidney stone disease?
People with kidney stones usually have severe back pain. This pain is also referred to as renal colic.
Renal colic usually occurs when the renal stones pass down in to the ureters, and compromise the urine outflow.
Renal colic is extremely severe and it is like somebody stabs you in the back with a sharp knife. And sometime the pain may radiate to the groin area.
The pain starts suddenly, and may fluctuate as the stone moves.
In addition, passing blood in urine, or hematuria,
Nausea and vomiting,
Low urine output,
Pain during urination,
High grade fever, are some other symptoms of renal stone disease.
Smaller stones may sometimes will not cause any symptoms.
So, how to treat renal stone disease?
Well, if the stones are smaller in size, they may spontaneously pass out when the person urinates.
However, this could be extremely painful.
Sometimes, medications are given to dissolve these stones, as well as to prevent further formation of new stones.
And if the person is having a urinary tract infection, antibiotics may be given.
For larger stones, surgical intervention may be needed.
Drinking a plenty of water,
Taking a low sodium and protein diet,
Uptake of food that are rich in citrate, which can inhibit the production of renal stones are recommended for prevention of kidney stone disease.
