Hugo is an online client who lives in USA and has been suffering from lumbar disc herniation in L5 S1 , lower back pain, Sciatica pain and hip pain for the last 3 years.

I have been facing with many clients recently who had a similar experience after visiting a therapist and they told them "Your hip Is Out of Alignment" "A patient once gravely informed me, as if sharing a dangerous secret, that a chiropractor had predicted his back pain by identifying a minor leg length difference ten years earlier.
The prediction was a warning: get your short leg fixed, or else you’ll be laid low by low back pain for sure!" Really ?

There are (at least) five major problems with these kinds of diagnoses:
1️⃣ Not only are structural explanations for pain generally unsupported by any scientific evidence, the last 25 years of research results mostly undermines them, often impressively.
2️⃣ Professionals can rarely agree on them (poor diagnostic reliability).
3️⃣ Most structural diagnoses are difficult or impossible to do anything about even if you can agree on them in the first place.
4️⃣ Patients are often given the alarming idea that the slightest crookedness is “serious” (and may require expensive therapy to correct).
5️⃣ Finally, biomechanics is obscuring more useful, new ways of looking at how pain works.

This is a common finding you may see on your MRI report. Should u be worry about it & does your rehab plan will be different ?
The short answer is no !! 💡
Our spine, or backbone, is made up of a series of bones called vertebrae. In total, we have 33 vertebrae, which are stacked on top of each other and separated by discs.
These vertebrae are grouped into regions: the cervical, thoracic, lumbar, sacral, and coccygeal regions. The lumbar region is located in the lower back and consists of 5 vertebrae, numbered L1 through L5.
These vertebrae are the largest and strongest in the spine, as they support most of the weight of the upper body. Between each vertebra, there are ligaments that hold them together and provide stability to the spine.
One of these ligaments is called the ligamentum flavum, which runs along the back of the spinal canal. Its main function is to connect adjacent vertebrae and maintain the space between them, which allows the spinal nerves to exit the spinal cord and travel to other parts of the body.
The ligamentum flavum is made up of elastic fibers, which allow it to stretch and bend as the spine moves. However, if this ligament becomes thickened or calcified due to aging it can cause compression of the spinal nerves and lead to pain or other symptoms.

Posterior Pelvic Tilt:
Posterior Pelvic Tilt exercises activate the intrinsic core stabilisers so you can practice to engage those muscles and the core is working and the lower back doesn’t take as much strain. The posterior Pelvic Tilt is an isometric hold for 3 to 5 sec and repeat it for 3 sets of 3.
Figure 4 stretch Prone:
The figure four stretch is one of the best stretches you can do for your body, regardless of the type of training you do. It targets the hips, lower back, and glutes, your body's largest and most powerful muscle group specifically your Piriformis muscle.
Try this stretch daily for 3 to 4 sets of 15 to 20-sec hold.
Cat Cow:
Cat Cow is a simple but effective exercise which can help you to reduce sciatica pain and improve your upper and lower back. You can try this exercise 3 times a day for 3 sets of 8 to 10 repetitions.
Hip Bridges,
With your feet flat to the ground and spread hip-width apart, drop your glutes slowly toward the ground. Lift your hips back up slowly and squeeze the muscles at the top of the movement. You can hold weights or a weighted bag on your hips to add more resistance.
Sciatica nerve glide:
This exercise can help you with the sciatica pain and nerve-tingling sensation. We presume you have the pain and tingling in your left leg. You’re going to lie down on your back and find a comfortable position. Bring up the left knee to your chest to the point that feels comfortable. Then you are going to extend the knee and bring your toe to dorsiflexion. Try to extend the knee to the point that feels comfortable and you start to feel the stretch. 10 Facts about lower back pain
In this video I'm sharing with you 10 facts about lower back pain that can help you to manage your lower back pain.
1) The spine is strong, robust and can be trusted.
The spine is a robust, adaptable structure, capable of safely moving and loading in a variety of postures. Common warnings to protect the spine are not evidence-informed and can lead to fear.
2) Persistent back pain is rarely associated with serious tissue damage.
Persistent back pain can be distressing and disabling. If you have had an injury, tissue healing occurs within 3 months. If pain persists past this time, it usually means there are other contributing factors. A lot of back pain begins with no injury or with simple everyday movement. These incidents may sometimes coincide with episodes of stress, tension, fatigue, inactivity or unaccustomed activity which make the back sensitive to loading and moving.
3) Getting older is not a cause of low back pain.
Although it is a widespread belief and concern that getting older causes or worsens back pain, research does not support this and evidence based treatments can help at any age.
4) Scans rarely show the cause of back pain.
Scans are only helpful in a minority of people. Lots of scary sounding things are reported on scans such as disc bulges, degeneration, protrusions, arthritis etc. Unfortunately these scans do not say that these findings are very common in people without back pain and that they don’t predict how much pain you feel or how disabled you are.
5) Pain with exercise and movement doesn’t mean you are doing harm.
When pain persists, it is common that the spine and surrounding muscles become really sensitive to touch and movement. The pain you feel during movement and activities reflects how sensitive your structures are, not how damaged you are. So it’s safe and normal to feel some pain when you start to move and exercise. This usually settles down with time.
6) Back pain is not caused by poor posture.
How we sit, stand and bend does not cause damage even though these activities maybe painful. A variety of postures are healthy for the back. It is safe to relax during everyday tasks such as sitting, bending and lifting with a round back – in fact it is more efficient.
7) Back pain is not caused by a “weak core”.
Weak “core” muscles do not cause back pain, in fact people with back pain often tense their “core” muscles as a protective response. This is like clenching your fist after you have sprained your wrist. Being strong is important when you need the muscles to switch on, but being tense all of the time isn’t helpful. Learning to relax the “core” muscles during everyday tasks can be helpful.
8) Backs do not wear out with everyday loading and bending.
The same way lifting weights makes muscles stronger, moving and loading make the back stronger and healthier. So activities, like running, twisting, bending and lifting, are safe if you start gradually and practice regularly.
9) Pain flare ups don’t mean you are damaging yourself.
While pain flare ups can be very painful and scary, they are not usually related to tissue damage. The common triggers are things like poor sleep, stress, tension, worries, low mood, inactivity or unaccustomed activity. Controlling these factors can help prevent exacerbations and if you have a pain flare up, instead of treating it like an injury, try to stay calm, relax and keep moving!
10) Injections, surgery and strong drugs usually aren’t a cure.
Spinal injections, surgery and strong drugs like opioids aren’t very effective for persistent back pain in the long term. They come with risks and can have unhelpful side effects. Finding low risk ways to put you in control of your pain is the key.

Too many people think that once you have a disc herniation (or disc bulge), that you’ve got it for life.
A recent meta-analysis (Zhong et al 2017) has shown that 2/3 of disc bulges spontaneously «heal» on their own! Many of them within a few months too, not years.
“The overall incidence of spontaneous resorption after lumbar disc herniation was 66.66% (95% CI 51% – 69%).”
In Conclusion they have highlighted:
“Because the overall incidence of disc de-herniation is now 66.66% according to our results, conservative treatment may become the first choice of treatment for lumbar disc herniation.”
