A disc bulge occurs when the outer layer of the intervertebral disc weakens, causing the gel-like inner material to push against it. This bulging can put pressure on nearby nerves, resulting in pain, numbness, and discomfort. So, how does the body initiate the healing process for such a condition?
👉This is where phagocytes come into play. Phagocytes are a type of white blood cell responsible for engulfing and digesting cellular debris, foreign substances, and pathogens. In the context of disc bulges, these cells play a crucial role in clearing up the damaged disc material and promoting the healing process.
1- 👉The healing process begins with inflammation, which is a natural response of the body to any injury or damage. When a disc bulge occurs, the body releases various chemical signals that attract phagocytes to the affected area. These phagocytes rush to the site of the injury, initiating the first step of the healing process.
2- 👉Once the phagocytes arrive at the damaged disc, they begin the process of phagocytosis, which is the engulfing and digestion of the damaged disc material. Phagocytes release enzymes that break down the debris, making it easier for them to remove it from the affected area.
3- 👉 As the phagocytes continue their work, removing the damaged material, the body starts to initiate tissue repair mechanisms. Other cells, such as fibroblasts, come into play to lay down new collagen fibers, strengthening the outer layer of the intervertebral disc.
4- 👉 Once the damaged disc material has been cleared and new tissue has formed, the phagocytes continue their role by reabsorbing any remaining cellular debris. This process helps in further cleaning up the area and promoting the remodeling of the disc, restoring its structure and function.

The cause of sciatica:
The vast majority of cases of sciatica have a spinal cause, such as disc herniation or rupture causing impingement of L5 or S1 nerve roots.
Other common causes of sciatica can include spinal stenosis, degenerative disc disease, and spondylolisthesis. There are also many non-spinal causes or sciatica, which are less common: These include piriformis syndrome, trauma, postoperative complications.
Sciatica is mainly diagnosed by history taking and physical examination of the patient by a physical therapist.
Sciatica sign and symptoms:
- Moderate to severe pain in the lower back, buttock, and down their leg.
- Numbness or weakness in their lower back, buttock, leg, or feet.
- Pain that worsens with movement; and may cause loss of movement control.
- Pins and needles” feeling in their legs, toes, or feet.
Exercises for sciatica pain relief and sciatica nerve-tingling sensation:
1- McKenzie exercises, back extension :
McKenzie exercises for low back pain are beneficial treatment for increasing flexibility of spine and improving the pain with better results in pain relief. McKenzie exercises are successful method for decreasing and centralising the pain and increasing spinal movements in patients with low back pain. If you suffering from L4 L5 / L5 S1 Disc bulges and Sciatica Pain this workout routine can help you to stay active without aggravating your pain and discomfort.
3- Posterior Tilt:
Lie down on a comfortable surface, such as a yoga mat or a firm mattress, with your knees bent and feet flat on the floor. Ensure that your back is in a neutral position, neither arched nor pressed flat against the floor. Start by engaging your core muscles. Imagine drawing your navel inward toward your spine without holding your breath. This action activates your deep abdominal muscles. Once your core is engaged, gently press your lower back against the floor/mat by tilting your pelvis backward. Imagine flattening your lower back and creating a small gap between your lower back and the floor. This is the posterior tilt position. Hold the posterior tilt for a few seconds while maintaining a relaxed breathing pattern. It's important not to strain or tense up other muscles while performing this exercise.Release the posterior tilt and return to the neutral position. Allow your back to rest in its natural curve. Repeat this movement for 3 sets of 5 repetitions.
2- Sciatica nerve glide: This exercise can help you with the sciatica pain and nerve-tingling sensation. We presume you have the pain and tingling in your left leg.
You’re going to lie down on your back and find a comfortable position. Bring up the left knee to your chest to the point that feels comfortable. Then you are going to extend the knee and bring your toe to dorsiflexion. Try to extend the knee to the point that feels comfortable and you start to feel the stretch. You may try this movement for 3 sets of 10 to 12 repetitions. This exercise will help you to reduced sciatica pain and nerve-tingling sensation.
2- Figure 4 stretch: This exercise is beneficial for sciatica pain and will help you to relieve pain from the buttock and improve your hip mobility. We presume you have sciatica pain in your left leg. You’re going to lie down on your back and find a comfortable position. You are going to cross the left knee, and then bring the left knee to your chest, as far as you start to feel the stretch. Avoid letting your lower back come off the ground. And hold this stretch for 30 sec. You may try this stretch for 3 to 4 repetitions.
5- Bird Dugs:
The bird dog is a simple core exercise that improves stability, encourages a neutral spine, and relieves low back pain. It strengthens your core, hips, and back muscles. It also promotes proper posture and increases range of motion. This exercise is suitable for people of all levels, including seniors, and it can be used to prevent injury, align your spine, and recover from low back pain.
If you don’t feel comfortable lying on your back, you can try the figure 4 stretch in the seated position and hold it for 30 sec for 3 to 4 repetitions.
If you suffering from sciatica, figure 4 stretch can help you to relieve pain from your buttock and improve your hip mobility.

Epidural corticosteroid injection for management of chronic lower back pain
Epidural steroid injections are commonly used to treat back pain caused by a herniated disc (slipped disc), lumbar radiculopathy, spinal stenosis or sciatica. Corticosteroids are strong anti-inflammatory medications and, when placed into the epidural space, can significantly reduce inflammation around an irritated nerve that is causing back and leg pain and discomfort.
Patients with several common conditions – including lumbar disc herniation, degenerative disc disease, lumbar radiculopathy, sciatica, lumbar spinal stenosis, may benefit from an epidural injection.
Epidural steroid injections work by delivering a potent anti-inflammatory to the site of nerve impingement in the spine. Injected medications may include steroids, local anesthetics, and saline, and they can vary in volume and concentration on an individual basis.
Research highlighted, Epidural corticosteroid injections can be recommended as additional therapy only in the acute phase of the conservative management of lumbosciatic pain.

According to the literature analyzed in this narrative review, there is no consensus on the use of ESIs for patients with chronic lumbar pain. ESIs seem to be effective in relieving symptoms in the short term and delaying surgery, while evidence of any long-terms benefits is still lacking. More studies are needed to better understand which patients could benefit more from epidural steroid injections.

Does Size of a Disc Herniation Matter?
In this study, we examined whether the size of a lumbar disc herniation (LDH) is predictive of the need for surgical intervention within 2 years after obtaining an initial magnetic resonance imaging (MRI) scan.
A total of 336 patients did not undergo surgery within 1 year of the LDH diagnosis. 185 had L4-L5 herniations (50.3%), and 169 had L5-S1 herniations (45.9%) . Patients who did not receive surgery had an average herniation size that occupied 31.2% of the canal, whereas patients who received surgery had disc herniations that occupied 31.5% of the canal on average.
After controlling for age, race, gender, and location of herniation through a logistic regression, it was found that the size of the herniation and the percentage of the canal that was occupied had no predictive value with regard to failure of conservative management, generating an odds ratio for surgery of 1.00.
Conclusions: The percentage of the spinal canal occupied by a herniated disc does not predict which patients will fail nonoperative treatment and require surgery within 2 years after undergoing a lumbar spine MRI scan.

A back spasm is a common condition that every individual experience at some stage in their life. However, it is noting to be worry and it should not stop you from doing your daily activities and your hobbies.
A back spasm is a sudden and involuntary contraction of the muscles in the back, often causing pain and discomfort. It can be caused by a variety of factors, including overuse of the muscles, sudden twist or turn, or sometimes due to a herniated discs or arthritis.
There is a series of steps that you should follow to help your body with the healing process. I have shared with you the POLICE and No HARM protocol to follow within the 1st 24 to 72 hours after your injury. I've shared with you why you should avoid complete bed rest after an episode of back pain. Beside that, I have shared with you series of exercises that you can do in bed in your acute phase of back pain these exercises will help you to relief your lower back pain, improve your mobility and reduce your overall symptoms.
1- Hip Stretch: helps to open up your hip muscle and improve your hip and lower back mobility.
2- Hip drop: helps to improve your back mobility and reduce your lower back pain and sensitivity.
3- The supine nerve glide exercise: is another way to mobilize and stretch the sciatica nerves in the body, particularly those in the legs and feet.
4- Thoracic rotation: simply helps you to reduce your lower back pain and reintroduce the rotation and twist to your hip and your lower back.
5- Back extension: is a great exercise to reduce pain in your lower back. If you suffering from disc bulges, sciatica pain, or simply having a back spams, back extension can reduce your pain.
