MIKE MENTZER a | 10 reps to FAILURE—which rep requires most intensity? Last One

presents episode 1161 | Mike Mentzer
Heavy Duty College podcast
Client: surprised that he had made such uninterrupted progress...
with heavy duty High Intensity Training [HIT]
"Should not be a surprise; has been happening for centuries;
it is logical and rational--
cause & effect relationship between
intense exercise & muscle growth
has been known for a long time
-even though many 'experts'
fail to grasp it
It is the laws of nature that dictate the CAUSES to enact that
will EFFECT muscle mass build up beyond normal...
Once these causes are understood; the task of building
muscle strength, while requiring HIGH INTENSITY EFFORT,
is really quite simple
Perhaps exercise scientist will
observe weightlifters & conclude:
it is volume that matters
"That is where I looked at first"
If volume is key: then more is always better...
20 sets: okay; 40: better; etc: Volume argument is absurd
It is counter-productive
Since quantity is not the right variable for muscle growth;
that leaves: INTENSITY
Example:
100# barbell, 10 reps to failure
-first rep: least intensity of effort [~8-10%]
-fatigues a little bit, so
-second rep: requires ~15% intensity
-each rep requires more intensity
-last rep: 100% intensity
Absolute Failure: shaking, gritting
teeth, barely get bar to top
Which rep required most intensity? LAST REP

Low Carb Down Under podcast
Between 2003 and 2018; 62 published studies...
31 had statistically significant results [on the graph]
-green bars: high fat/low carb
In EVERY CASE: High Fat group lost MORE WEIGHT than low fat
Most doctors STILL promote
low fat for weight loss
Example: handout from
doctor recommending
low fat for weight loss
Some doctors won't believe a patient who has HEALED himself...
Example: 71 year old man--reversed diabetes, fixed blood sugar,
dropped 5 meds, put inflammatory bowel disease into remission!
-ALL THRU DIETING
Shouldn't his endocrinologist have been happy?
BUT, in letter to Paul
-admitted diet helped
THEN recommended
returning to PREVIOUS
DIET that caused the problems in first place!
[patient told him he had already tried this diet]
-and recommended taking statins
Light at end of tunnel: American College of Cardiology
[once super strong anti sat fat] in recent review paper:
ACKNOWLEDGED that EVIDENCE does not support
restricting saturated fat intake
RECOMMENDS
-dairy; meat; eggs
be FREELY CONSUMED
3 authors were on 2005 committee
for USA Dietary Guidelines that had
promoted grains, fruit, veggies
We need
1-dietary guidelines to catch up with this latest science
2-doctors to wake up to this better approach
Health Professionals Be Aware:
Page 2 of guidelines:
"These recommendations are
ONLY for healthy individuals;
NOT FOR THOSE REQUIRING
SPECIAL DIETS"
For diabetes or obesity:
government guidelines

2 huge studies hoping to prove polyunsaturated fats
[low cost darlings of food industry] were healthy...
results WERE HIDDEN because they were DISAPPOINTING
to the funders!
BMJ study DEMOLISHING MYTH to avoid sat fat due to LDL
Results:
highest LDL = LIVED LONGEST
systematic review of ALL
STUDIES [68k participants]
16 of 19 studies found same
result: high LDL=long life
Each line: one study; ea column: LDL level
-left column: lowest LDL quartile; right: highest
-numbers: relative risk of dying
[less than 1 = reduced chance of death]
Highest LDL = reduced chance of dying
Example: low LDL: 1; high LDL: .66 meaning that
highest LDL had 34% lower chance of death
Some claim it is really 'reverse causality'
'illness lowers LDL...those with low LDL were
already ill'
Some claim it is really 'reverse causality'
'illness lowers LDL...those with low LDL were already ill'
This argument fails on 3 levels...
1-studies conducted for longer than 2 years each
2-even patients with terminal disease excluded: no change
3-if exclude lowest quartile
[highest LDL] results
still SAME
"This review is robust
dismissal of results
is scientifically dishonest"

PAUL MASON b- | Sydney Heart—huge study..veg oil group: DEATH RISK up 62%
http://www.DoctorsToTrust.com
@doctorstotrust3552
presents episode 1156 | Dr Paul Mason
Low Carb Down Under podcast
I gave a nutrition test to junior doctors; they were outscored
by the receptionist...public beware!!
-don't be guided by the 'food star' system [Australia]
-it is completely controlled by food industry
Example: cereal with 17% SUGAR is given 5 stars
-salt-cured salmon: 1.5 stars
Example of misleading publishing of nutrition research...
2006 Women's Health Initiative RCT study of 48k females
-designed to confirm idea that: low fat diets were healthy
-2 groups: low fat & regular...followed for 8 years
Lead investigator announced proudly that the study showed
need for low fat diets!
Results showed nothing
of the sort...
To see the real results,
had to go to page 661
One statistically significant result:
low fat group who had history of heart issues, had 26% higher
risk of complications--like heart attacks
Q--how was this finding, of this highly public, expensive study
kept hidden from public view??
Q--how did researchers
claim that this study
supported extreme
low fat diets??
Results were corrupted
Sydney Diet Heart RCT study: men who had had heart attacks:
-replacing saturated fats with polyunsaturated fats [PUFA]
-results originally were NEVER PUBLISHED
-until researcher discovered original data, hidden in basement,
published it 40 years later [2013 in BMJ]
Results:
low sat fat [high PUFA]
group: INCREASED
RISK OF DEATH 62%

Low Carb Down Under podcast
Sydney Diet Heart RCT study--when actually published in 2013
faced barrage of criticism...
Main criticism: 62% increase in death risk due to transfat
[they said: 'PUFA group ate margarine high in transfat']
-that is ALL they had against the published results
"But, margarine used in
PUFA group had no
transfat!"
"Pastries with transfat
were DISCOURAGED
for PUFA group"
"Safflower oil was used [no transfat]"
Summary: transfat in PUFA group was reduced...
TRUTH: polyunsaturated fats, as RECOMMENDED by current
government dietary guidelines, was CULPRIT in deaths up 62%
"This was long term, randomized, control trial--hidden 40 years"
Minnesota Coronary Survey, 1973, double-blind, RCT, 9000
men & women...High sat fat vs High PUFA diet
NEVER PUBLISHED until data found, published 2016--
hard to believe the corruption!
High PUFA diet increased risk of dying
Before his death, lead author
asked why it was not published...
He said: 'because results were
disappointing!!

PAUL MASON a- | “Your doctor may be unaware of corruption in ‘expert-reviews’
http://www.DoctorsToTrust.com
@doctorstotrust3552
presents episode 1155 | Dr Paul Mason
Low Carb Down Under podcast
Doctors provide poor information, at times, but don't
deliberately deceive...
-many have trouble separating fact from fiction
Email from colleague physician, that gave feedback on her
use of statins with her diabetic patients
-she was being pressured to
prescribe statins to every
diabetic patient!
-despite her belief that many
of her patients do not need it
-she looked for scientific papers
to clarify this
She found an 'expert review' paper which strongly promoted
statins for diabetics. It CLAIMED that statins would:
Help Patients Live Longer
6 references in support of the claim
-checked all 6 [if the claim is better mortality, studies must
report on mortality]
3 did not report mortality
3 on mortality did not get
statistically significant
results
[findings could have
been by random chance]
Very poor research when determining a medicine's worth...
Women's Health Initiative: found statin use can increase risk
of diabetes 71%
-Statin use increases risk of getting diabetes &
-risk of poor sugar control for diabetics
"Evidence used to support
the 'experts' claim' was
horrific"
This weak, corrupt paper
has been cited 69 times
in medical literature!
Problem: 'eminence-based' medicine relying expert opinion, not
on scientific data
-those in authority at venerable institutions exert undue
degree of influence over general opinion
"It is the direct ENEMY of evidence-based medicine"
Which is: Critical Appraisal
of the best available science
The doctor who sent the email
was trying to do the RIGHT
THING--and knew enough not
to trust the opinions of
these 'experts'
Many doctors are not aware that they have been
-misled & fed falsehoods in their training
"Most doctors still BELIEVE Hook Line & Sinker the
nutritional FALSEHOODS they were taught"

stomach pain; celiac disease; anxiety; brain fog
Dr Peterson, 57, harder to get under control...
-extreme depression & anxiety
muscle weakness; fatigue
brain fog; weight gain; GERD;
psoriasis

What if your elimination diet does not work?
-if you have tried carnivore and still have issues;

-went to restaurant in Florida: had to check EVERYTHING
-grill had lingering things, so got mild reaction
-don't really eat out
-depression is triggered
pretty easily
-so is Dad's
Listen to your body!!
Typical example: at doctor, tell about a symptom, response:
'everyone has that' so you set it aside and try to ignore it
-not necessarily normal: 'everyone gets 3pm slump...NOT'
-if really, really sensitive, go 100% all in for at least 6 weeks

Plant-based diets missing many essential nutrients...
-B12, D3, K2, Retinol [A], DHA, EPA
-beta carotene: 45% people cannot convert to vitamin A
-Plants that have K only have K1, most not converted into K2
-These nutrients are ONLY AVAILABLE IN MEAT, EGGS, FISH
"Any nutritionist who advocates
for plant-only diet is misleading...
These nutrients cannot come
from plants"
But, many university programs for
nutrition DO ADVOCATE for
PLANT ONLY EATING
Dietitian associations in Australia [and in US & elsewhere]
promote plant-based diets...
"Even say well-planned vegan diet is best, as long as you
supplement for missing nutrients"
They say: Low carb diet is a fad diet with no scientific evidence
Yet, thousands of human studies show
medical benefits
"Recently had to change this due to
keto diets are now part of T2D
treatment schedules
Dietitian Association Australia
listed 4 studies 'showing' vegan diet was best...
Each of those 4 studies were funded & done by either
-Sanitarium Foods or SDA Church
Majority of plant-based positive studies come from
compromised sources
-food companies
Example: Coca-Cola spends 11x more
than USA's NIH on studies
Also
-Nestles, Pepsi, Kelloggs
"Studies are biased due to this funding

-mostly fatty meat
-we have done this forever
-should continue this
-avoid 'foods' that did not
exist 50 years ago
[highly processed foods, sugar

[very robust design as each group
becomes control and virtually all
confounding variables are accounted
for and eliminated]
Is this representative of all runners? probably very close...
-30% of runners on high carb will become prediabetic in 30-40s
In the 70s, we trained really, really hard
-finished the marathon in 3 hours...everyone left
-no one waited for those in 4-6 hours
-everyone was LEAN [few carbs]
Now: see people finish in 6 hours
"They are a metabolic disaster due to
carrying excess fat. They believe
they are healthy due to so much
running--that is not true"
Some may be able to eat carbs and remain healthy due to
excess training.
-if have visceral fat, wide waist then you will be prediabetic
"Carbs will kill you.
This from the sports doctor who started the carb loading
craze and invented first sports gel!
I know, because I watched my father
die of T2D and as a doctor,
I did not know what to do"

TIM NOAKES d | Before carb era: all great athletes were fat-adapted: burned fat for high performance
http://www.DoctorsToTrust.com
@doctorstotrust3552
presents episode 1133 | Dr Tim Noakes
Levels podcast
When you stop eating carbs...your muscles full of fat,
will burn fat
2 years ago, studied low carb, really good athlete...
-did 100k time trial--from moment got onto bike,
-burning 1.7g fat/minute
If athlete carb-adapted, would get nowhere near that level
-carb athlete starts at .4g/min and
takes hours & hours to get near 1.7g
"It is the content of muscle glycogen
determines how much carb & fat you
burn
I went back to original 1967 Scandinavian studies...
checked low carb group: they could only exercise for 1 hour, but
had highest level fat oxidation ever recorded--
researchers did not notice it because they focused only on carbs
LOW CARB GROUP WERE NOT FAT ADAPTED
-yet did burn fat for one hour
"My interpretation: body is designed to
be fat-adapted...it only deviates from
this when eating high carb diet"
-our natural state is to burn fat
Dr Louise Burke: high carb athletes...
in 5 days of low carb: burning 1.6g/min fat
Why do we always burn some glucose...even when fat-adapted
for decades?
Some glucose is obligatory for brain
-so, blood glucose oxidation goes up during exercise to preserve
some glucose for brain
[we know from Dr Mason, Dr Bikman
that brain prefers ketones for fuel, but
also that parts of brain need glucose]
-some obligatory glucose may be for
kidneys or other tissues--even muscle
but a tiny, tiny amount
"Used to think glucose was predominant for muscle"
Another study, 1986, Dr Edward Coyle, showed ingesting
carbs during exercise could improve performance...
-starved athletes 12 hours before study to get model to work
Dr Noakes reviewed data of metabolic state of athletes
-they did not report fat oxidation rates
-did report carb oxidation rates
"When these athletes took carbs to
boost performance, they were burning
bat at 1.2g/min--highest value ever
reported at that time!
Why? Were fat-adapted at the time.
-this was before carb loading craze hit

TIM NOAKES c | Athletes: carb load? Brain tells muscle burn baby burn carbs
http://www.DoctorsToTrust.com
@doctorstotrust3552
presents episode 1132 | Dr Tim Noakes
Levels podcast
Difference between glucose and glycogen?
-we think: whether glucose comes from bloodstream or muscle
the regulation is the same...that is TOTALLY FALSE
During exercise, muscle glycogen depletes in linear manner
according to duration, therefore
the exercise is regulating how much
glycogen is being used
IT IS ALWAYS GOING DOWN
The glucose coming into the blood
from liver is circulating to muscle,
being used by muscle, but regulation
is totally different...
The glucose coming into the blood
from liver is circulating to muscle,
being used by muscle, but regulation
is totally different...because blood glucose oxidation INCREASES
But, don't want that to happen, due to
poor liver more & more depleted of
glucose & glycogen
-more difficult to produce glucose
-muscle says 'I don't care, I need more'
-liver runs low and blood glucose falls
Brain, which needs glucose, won't let liver die..
-as blood glucose drops the power output for athlete drops
begin to get fatigued--many studies have shown:
-if they take in glucose, in few minutes they are back to normal
Shown:
-blood glucose is critical, body burns it
-body could burn fat for everything
-but in these studies it does not
WHY?
-studies where athletes are measured
at rest: 50% of calories are from carbs
This does not make sense...carbs are jet fuel and so,
why is body wasting it while resting?
Dr George Cahill, 1971 paper:
"First rule of metabolism is:
-body regulates blood glucose level
-keeps it within narrow band
If it goes outside of range, body reacts
dramatically to correct it.
-dumps excess into muscle
Brain knows too much in muscle, but
knows you always intake more carbs...
-so it burns the glucose due to your routine!

TIM NOAKES b | Body can adapt from carb to fat burning: At ALL LEVELS OF INTENSITY TRAINING
http://www.DoctorsToTrust.com
@doctorstotrust3552
presents episode 1131 | Dr Tim Noakes
Levels podcast
Real learning from this study:
-after each race: measured oxygen & RQ [respiratory quotient]
-to measure carb & fat oxidation
"The more sprints they did, the higher fat oxidation went up!"
-VO2 max was 86%
-but, text books say: at 85% VO2 max
will burn no fat
-this was highest fat oxidation in
history at the very point they were
not supposed to be burning fat!
Shows that muscle glycogen has no obligatory role during
exercise--even at these extremely high intensities...
Look at original studies, Scandinavia, 1967, claiming that there
is an obligatory role for muscle glycogen...
-at point of exhaustion, all had low
muscle glycogen, blood glucose level
was incredibly low
Scandinavian studies...
-at point of exhaustion:
-high carb; mixed carb; low carb athletes
-big differences in muscle glycogen; fat oxidation; carb oxidation
[which doesn't fit the model]
Not different?
-blood glucose concentration
"This was the LIMITING FACTOR"
More studies showed:
-take high carbs during or before exercise:
you sub a bit more carb for a bit less fat
-the kilojoules are absolutely MATCHED, so
"If burn extra 8kj/min of carbs;
you burn 8kj/min of fat less
We have been told, since sports drink
industry got involved [1980s]...
They said: that is the difference!
you burn extra carbs during race that makes you perform better
"But, how does burning 8kj/min carbs
[only 15% of energy] so special?
-you can make that up burning fat
-it is now proven that you CAN
-but, in this case, all groups were
hypoglycemic
-it is blood glucose that is obligatory
fuel, not muscle glycogen
What this shows us, with new measurement understanding...
-body can adapt from carb-reliance toward fat-reliance
AT ALL INTENSITIES OF EXERCISE
-was believed [and promoted by sports drink funds] that
above certain threshold [85% VO2max]
you have to run on carbs

with Josh Clemente: Dr Tim Noakes:
-started sports science in 1981...
-thought: hypoglycemia developed during endurance exercise
-people encouraged not to drink water during marathon
-had examples of racers who benefited
by taking carbs mid-race
"I thought muscle glycogen depletion
causes fatigue during exercise"
-wrote book: Law of Running, which
promoted carbs, carbs, carbs
last edition 2002
2010: learned about low carb eating...
-within 4 months dropped lots of weight, running improving
"So, the main doctor promoting high carb, now was low carb"
-wrote some articles and immediately lost all funding
-had to retire from university
due to promoting 'dangerous diet'
Had to go to court to face lawsuit for
claiming diet could reverse T2D
-fought case for 4 years
-during this time reversed own T2D
2018 won the case
"I learned whole body nutrition"
With Philip Prince, did study of 5k runs, 2 groups:
-low fat v low carb
-showed that at 85% of VO2max,
switched from burning carb to burning fat
Next: 1k race
result: no difference between groups
"People will say: of course, due to
their glycogen in short distances"
Let's do 6 times 800m sprints
-measured oxygen consumption & metabolism
Result:
NO DIFFERENCE IN PERFORMANCE bet low carb & low fat
"What should have happened, according
to establishment protocol:
after 3 sprints , low carb group
performance should have gone down
due to no muscle glycogen left"
-that did not happen
-both groups performed the SAME
Real learning from this study:
-after each race: measured oxygen & RQ [respiratory quotient]
-to measure carb & fat oxidation
"The more sprints they did, the higher fat oxidation went up!"
-VO2 max ws 86%

Insulin Resistance
Compromised ability to use glucose as fuel:
-migraines
-epilepsy
-depression
-some instances of autism
Most efficient way to fatten a human:
include abundant amounts of food
that increase insulin
Insulin significantly reduces metabolic rate:
accounting for 30% of fat gain
Control carbs
-if it comes in a bag or box with a bar code...be careful
-sugar is terrible
-including fruit juice and smoothies
Prioritize protein
-optimal daily intake: 1.5g protein per kg ideal body weight
[daily: 100 - 150g minimum for average adult]
Stop fearing fat
Test what you learn
-try it for a month
-it should be sufficiently practical to follow indefinitely

Low Carb Denver podcastPrimary prevention:
Primary prevention of heart disease...
Collecting all of the data yields: Primary Prevention
-does not help women in mortality reduction whatsoever
Define: primary prevention is someone with no cardiac event
-if have CAC above 0, but no event,
still primary prevention patient
If CAC score is 0, studies show
-up to a decade with statins: no benefit
"In right clinical setting, don't need statin"
If you fall outside this category...say: middle-aged man taking
statins for primary prevention. Take 100 of these, the benefit
of statins in mortality reduction? 0.1%
[99.9% for that year: no benefit]
Physicians are told: relative risk reduction
is how to measure statin benefit
Dr Malcolm Kendrick would say:
It is like saying, if winning lotto is 1/15m
"I will increase your odds by 100%"
[increases odds from 1 to 2 in 15mil]
In actual trials: presented as 30% better
-when actually: just 0.1% in this group
Dear doctor: Question: On this relative risk basis, you are
going to give this drug to patient to take for 30 years!!?
-comfortable making that decision alone?
-want to empower patient to decide?
-want to discuss side effects?
"This is where our medical profession
IS LACKING in CRITICAL THINKING
for themselves"
-rely too much on opinion-leaders

presents episode 1124 | Dr Nadir Ali
Low Carb Denver podcast
Animal studies: using PCSK9 inhibitor-like approach
results: higher risk of diabetes & visceral obesity &
higher fat in heart
WHY? It removes LDL receptor in liver & pancreas, that was
sucking up LDL from bloodstream...
LDL particle is not just cholesterol filled,
but is fat filled, so: more receptors &
fat is removed from circulation, giving
fatty liver & fatty pancreas
People born with PCSK9 DEFICIENCY: they have higher risk:
-diabetes; higher weight; more visceral obesity
Fourier PCSK9 trials, 28k patients: half control; half: Repatha
results: Repatha group dropped LDL: to 30mg/dL [.8 mmol/L]
Control: 90mg/dL
OUTCOME: no mortality difference!
"Abject failure of the drug"
SUM: PCSK9 inhibitor: higher risk
-diabetes, visceral obesity
Human genetic trials: same result
"Do not tell patients this drug beneficial"

presents episode 1123 | Dr Nadir Ali
Low Carb Denver podcast
This grass roots MOVEMENT of evaluating ONE'S OWN HEALTH
-thru metabolic parameters & conferences like LowCarbDenver
-has leveled the playing field between physicians and
consumers of medicine
This will change the paradigm re what
is important in cardiac health &
protect doctors like Dr Ali
The movement will focus on
-IR; inflammation; quality of lipoproteins
-NOT on levels of LDL
Example: 100 patients who have LDL
above 250, but CAC score = 0 for 3 years
Also, many patients with LDL above 250 with NO CARDIAC
EVENTS in 10 years...
This will continue to improve as research in this field grows
PCSK9: protein made by liver that binds to LDL receptor in the
liver: to suck up the LDL [the other good cholesterol]...
-it destroys the receptor, LDL remains in circulation
[LDL is higher, which bothers the Lipid Theory of Heart Disease]
So: PCSK9 INHIBITORS invented to
reduce LDL in blood by blocking PCSK9
from eliminating LDL receptors...
this lowers LDL
Animal studies: using PCSK9 inhibitor-like approach
results: higher risk of diabetes & visceral obesity &
higher fat in heart

practice and began following low carb: no patient has had a
cardiac event due to:
following a fasting diet; low refined carb diet; exercise...
And have not seen an increase in
cardiac events in those who stopped
taking statins"
-if this were early 2000 it would be
very different
-because of Dr Tim Noakes and others
I am protected in this approach
"Over past 10 years, I have been referred often due to
my Youtube channel or LowCarbDenver talk [viewed 1mil+]
-new patients are already motivated to try this approach
-they may be doing better due to their self-selection
than others who prefer the standard
establishment approach to meds"
Pushback from your hospital?
-in beginning I was reported to med staff
'patient came to hospital with cardiac
event while not taking statin'
My reply: you have had 10 patients with
cardiac events while on statins...
Cardiac disease has so many variables of causation...
-lipid profiles; sleep; stress; IR; inflammation
If I have cardiac event tomorrow...don't assume, because
I follow low carb; have high LDL; low TG; high HDL,
that the event is due to high LDL
-I have high stress on the job
-loss of sleep due to work...
"Be cautious when considering heart
disease is caused by one variable"

presents episode 1121 | Dr Nadir Ali
Low Carb Denver podcast
Dr Nadir Ali, cardiologist: with heart patient & statins...
-in secondary prevention [already had stint, bypass surgery, or
had myocardial infarction]
-best trial of statins was Forest: 1994, Scandinavia,
-funded & managed by Merck
Results: degree of benefit [mortality risk]
-in statin group v control:
saved 0.6% lives per year
-99.4% had no benefit
"in my opinion, very small improvement"
-brings increased risk of myopathy &
worsening of diabetes, ED"
Dr David Diamond researched this sub-group that benefited
from statins: they were severely insulin resistant
-had high TG & low HDL--only group who benefitted!
"When I consider a drug that a patient will take for 20+ years...
I want to explain degree of benefit
in absolute risk reduction
[NOT RELATIVE RISK,
which exaggerates the benefit]
Patient decides on drug or on lifestyle
changes: fasting, nutrition, exercise"
Many patients decide statin benefits
-are not what they had thought
