Chronic obstructive pulmonary disease, or COPD, is a PROGRESSIVE inflammatory lung disease characterized by INCREASING breathing difficulty. Other symptoms include cough, most commonly with mucus, chest tightness and wheezing. COPD develops as a result of LONG-TERM exposure to irritants such as smoke, chemical fumes or dusts, and may go UNNOTICED for years. Most people show symptoms after the age of 40 when the disease is already in its advanced stage.
The lungs consist of millions of air tubes or airways, called bronchi and bronchioles, which bring air in and out of the body. These airways end with tiny air sacs - the alveoli - where the gas exchange process takes place. REPEATED inhalation of irritants results in a CHRONIC inflammatory response which brings in a large amount of defensive cells along with inflammatory chemicals from the immune system. Inflammation of the airways causes them to thicken and produce mucus, NARROWING the air passage – this is known as CHRONIC BRONCHITIS. Inflammatory chemicals also dissolve alveolar walls, resulting in DESTRUCTION of the air sacs – this is EMPHYSEMA. COPD is, basically, a COMBINATION of these two conditions.
Tobacco smoking is accountable for about 90% of COPD cases. These include current, former smokers and people frequently exposed to second-hand smoke.
Extended contact with harmful chemicals such as fumes from burning fuel or dusts, at home or workplace, may also cause COPD.
Genetics has been implicated in a small number of cases. Notably, a condition known as alpha-1 antitrypsin deficiency, or AAT deficiency, has been shown to increase risks for COPD and other lung diseases. AAT protein protects the lungs from damaging effects of enzymes released during inflammation. Low levels of AAT make lung tissues more vulnerable to destruction when inflamed. While people with AAT deficiency may develop COPD even WITHOUT smoking or exposure to harmful irritants, AAT deficient smokers are at MUCH greater risks.
COPD is diagnosed based on symptoms, history of exposure to irritants and lung function tests. The major test for COPD is SPIROMETRY, in which the patient is asked to blow into a tube connected to a machine – a spirometer. Spirometry evaluates pulmonary functions by measuring the volume and the speed of air flow during inhalation and exhalation.
There is no cure for COPD but treatments can relieve symptoms, prevent complications and slow down progression of the disease. The first and most essential step to treatment is to stop smoking and/or improve air quality at home and workplace. These are also the most effective measures in preventing the disease.
Other treatments include:
-Medication: bronchodilators are used to widen the airways; steroids to relieve inflammation.
-Vaccination against flu and pneumococcal pneumonia: this is to prevent serious complications COPD patients may have with these respiratory infections.
-Supplemental oxygen: this can improve quality of life provided that the patient no longer smokes.
-Breathing exercises and other therapies as part of a pulmonary rehabilitation program.
-Finally, surgery may be performed for severe cases when other methods fail. Surgical procedures include bullectomy, lung volume reduction surgery, where damaged parts of the lung are removed; and lung transplant, where the entire diseased lung is replaced with a healthy lung from a deceased donor.

✔. The most important diagnostic tool is a detailed history focused on the meaning of “dizziness” to the patient. The simplest provocative test for vestibular dysfunction is rapid rotation and abrupt cessation of movement in a swivel chair.
✔. This always induces vertigo that the patients can compare with their symptomatic dizziness. A final provocative and diagnostic vestibular test, requiring the use of Frenzel eyeglasses (self-illuminated goggles with convex lenses that blur out the patient’s vision, but allow the examiner to see the eyes greatly magnified), is vigorous head shaking in the horizontal plane for about 10 s.
✔. The evaluation depends on whether a central etiology is suspected. If so, MRI of the head is mandatory. Such an examination is rarely helpful in cases of recurrent monosymptomatic vertigo with a normal neurologic examination.
✔. Treatment of acute vertigo consists of bed rest (1–2 days maximum) and vestibular suppressant drugs such as antihistaminics (meclizine, dimenhydrinate, promethazine), tranquilizers with GABA-ergic effects (diazepam, clonazepam), phenothiazines (prochlorperazine), or glucocorticoids.
✔. If the vertigo persists beyond a few days, most authorities advise ambulation in an attempt to induce central compensatory mechanisms, despite the short-term discomfort to the patient. Chronic vertigo of labyrinthine origin may be treated with a systematized vestibular rehabilitation program to facilitate central compensation.
✔. Prophylactic measures to prevent recurrent vertigo are variably effective.
✔. Antihistamines are commonly utilized but are of limited value. Ménière’s disease may respond to a diuretic or, more effectively, to a very low salt diet (1 g/d).
✔. Recurrent episodes of migraine-associated vertigo should be treated with antimigrainous therapy .

Thank you for visiting this video segment of Dr. AFib. I'm Dr. Morales. February is heart month. So I wanted to do some video features about how atrial fibrillation affects your heart as a whole. And not just talk only about atrial fibrillation. So In this video segment, we are going to talk about atrial fibrillation and valvular heart disease.
Now in your body, you have four main heart valves, two in the right, two on the left and the ones on the left more can affect having atrial fibrillation. First I will touch on aortic valve heart disease. The aortic valve is the last valve in your heart as blood is going out of your body to the heart to the rest of your body and is more directly connected to the left ventricle.
People when they have stenosis of the aortic valve, which is relatively common especially as people age or insufficiency, which is a leaky aortic valve. Aortic valve disease more indirectly causes atrial fibrillation. The way it works is that the aortic valve disease, first affects the left ventricle, which is the left lower chamber of the heart then can affect the atrium, which is the upper chambers of the heart. There's a little more indirect effect of how it can eventually cause atrial fibrillation.
However, what I mainly wanted to focus on was the mitral valve, the mitral valve is the one that is directly connected to the left atrium and has a more direct effect regarding causing atrial fibrillation.
Now many people have mild forms of mitral valve disease such as mild mitral valve insufficiency or mitral valve prolapse and those are unlikely to cause atrial fibrillation. However, as these diseases get more moderate or even severe, it can more directly cause atrial fibrillation. What happens is that they put a lot of direct pressure on the left atrium. The left atrium becomes more dilated, and stretch may develop more scar tissues, and that's what eventually leads to atrial fibrillation.

The mitral valve serves to ensure one-way blood flow from the left atrium to left ventricle of the heart. It opens when left atrial pressure is higher than left ventricular pressure, allowing blood to fill the left ventricle; and closes when the ventricles contract, to prevent blood from flowing back to the atrium. The mitral valve has 2 flaps, or leaflets, supported by a fibrous ring.
Mitral stenosis occurs when these leaflets thicken and become stiff, causing the valve opening to narrow, reducing blood flow. As a result, blood volume and pressure in the left atrium increases, and, over time, this may have several consequences.
First, the left atrium enlarges and becomes a risk factor for developing atrial fibrillation, a condition in which the atria beat rapidly and irregularly. The atrium quivers rather than contracts, and does not empty completely into the ventricle. Ineffective pumping causes the blood to stagnate, facilitating the formation of blood clots. These clots may then pass into the bloodstream, get stuck in small arteries and block them, resulting in stroke and other problems.
Second, because the left atrium receives blood from the lungs, pulmonary pressure may increase, causing secondary pulmonary hypertension, which in turn, may lead to right ventricular heart failure, as well as tricuspid or pulmonary valve regurgitation.
Mitral stenosis is most commonly caused by rheumatic fever, a complication of untreated strep throat or scarlet fever during childhood. For this reason, it is most prevalent in developing countries where rheumatic fever is more common. Rarely, mitral stenosis may develop with age, as a result of accumulated calcium deposits on the valve. Mitral stenosis can also be congenital.
Symptoms progress slowly, over years or even decades, so patients may not be aware until atrial fibrillation or heart failure develops. Symptoms may appear or worsen with increased heart rates, such as during exercise or stress. Women may suddenly discover they have the condition as they become pregnant.
Mitral stenosis produces a characteristic heart murmur that can be heard with a stethoscope. Diagnosis is confirmed with echocardiography, which uses ultrasound to visualize cardiac structures and blood flow. Echocardiography also helps determine the severity of the disease by measuring the mitral valve area. ECG recordings and chest X-ray may show signs of left atrial enlargement.
Because most cases of mitral stenosis are caused by rheumatic fever, prompt treatment of strep throat with antibiotics effectively prevents both rheumatic fever and mitral stenosis.
Treatment is not needed for asymptomatic patients. Patients with mild symptoms may be treated with diuretics to reduce blood pressure; beta-blockers or calcium channel blockers to control heart rates; and anticoagulants to prevent blood clots.
Valve repair or replacement surgery may be indicated for moderate to severe cases.
In percutaneous valvuloplasty, a catheter with a balloon is threaded through a vein and into the heart. The balloon is inflated to widen the opening of the valve, then deflated and removed.
Patients with heavy calcification may require open heart surgery to repair the valve. Valve replacement is considered when repair is not possible. Artificial valves can be mechanical or bio-prosthetic. Mechanical valves last longer but usually require life-long anticoagulation to prevent formation of blood clots.
