A renowned pulmonary critical care specialist who has run multiple intensive care units, Dr Pierre Kory stood firm in the face of relentless criticism and the systematic dismantling of his career following his stance on the medical response to Covid-19. He asserted consistently that effective treatments were available, saying corticosteroids and other therapeutics were effective in the treatment of the virus. Dr. McCullough was invited for an interview by Lara Trump, the daughter-in-law of President Trump. Ms. Trump is very well-spoken, and asks some very good questions. It is a brief interview, and not nearly as detailed, main focus is on the virucidal nasal/oral washes and early treatment but it is quite remarkable that a member of the Trump family was interested in interviewing Dr. McCullough. Dr. Zelenko used to think of this interview with John Leake (dated July 7, 2021) as one of the best interviews that he had ever done. The standard technique for solving equations with radicals is to square both sides of the equation as many times as necessary to eliminate all radicals. Because the procedure violates logical equivalence, it results in extraneous solutions that do not satisfy the original equation, making it necessary to check all solutions against the original equation. We propose alternative solution procedures that are rigorous and simple to execute where the extraneous solutions can be identified without verification against the original equation. Re-analysis of policy-shaping studies on hydroxychloroquine and ivermectin reverses original findings to yield significant benefits. Review in the context of regulatory decisions on vaccines. Abstract: It was with skepticism about the efficacy of hydroxychloroquine (HCQ) in Covid-19, that we read a paper reported in NEJM that was one of only two substantive papers cited by FDA in its revocation of the EUA for HCQ in June 2020. This was a study of postexposure prophylaxis (PEP) in persons experiencing a high or moderate risk exposure to Covid-19. Subjects enrolled in the study online and received HCQ or a folate placebo with the goal of preventing development of disease. Data presented in the supplemental appendix suggested, surprisingly, that the drug may have some efficacy in certain subpopulations. After contacting the PI, he pointed out a possible benefit of very early intervention. Exploring this observation, an examination of the raw dataset suggested that the intervention lag had likely been underestimated. After further data requests, we established that $52\%$ of subjects had received drug later than the overnight delivery assumed by the investigators. Correcting for this error yielded a $42\%$ reduction $(p=0.044)$ in Covid-19 compatible disease associated with HCQ given $\leq 3$ days from exposure. We found a similar problem in a companion study performed by the same team under the same logistical arrangements addressing early symptomatic subjects. We were unable to obtain corrected intervention lag data to determine if effects could be observed that were greater than the $20\%$ reduction of symptomatic Covid-19 reported originally. We also re-analyzed a study of early treatment using ivermectin (IVM) reported in JAMA in March 2021. The paper reported several execution errors or changes, notably the dosing of some placebo subjects with active drug, and the use of different types of placebos in different trial phases. Obtaining the raw dataset, accounting for these issues yielded a $56\%$ reduction $(p=0.033)$ of residual Covid-19. We will discuss these studies in the context of significant decisions made during the pandemic, particularly in light of recent decisions made by FDA and CDC regarding the Covid-19 vaccines. Frequentist and Bayesian analysis methods for case series data for the early outpatient treatment of COVID-19 Abstract: Dr. Zelenko proposed an early outpatient treatment for the SARS-CoV-2 pandemic that has evolved into Dr. McCullough's more detailed sequenced multidrug therapy protocol, corroborated with additional case series by Dr. Procter and Dr. Raoult. We introduce mathematical techniques for conducting rapid statistical analysis of these case series data that can be used to make emergency decisions, on whether to deploy these treatment protocols. In particular, we use the Sterne interval and the Bayesian factor to compare the observational case series outcomes on hospitalizations and death with treatment against the expected hospitalization and mortality rates without treatment. In the two-layer quasi-geostrophic model, the friction between the flow at the lower layer and the surface boundary layer, placed beneath the lower layer, is modeled by the Ekman term, which is a linear dissipation term with respect to the horizontal velocity at the lower layer. The Ekman term appears in the governing equations asymmetrically; it is placed at the lower layer, but does not appear at the upper layer. A variation, proposed by Phillips and Salmon, uses extrapolation to place the Ekman term between the lower layer and the surface boundary layer, or at the surface boundary layer. We present theoretical results that show that in either the standard or the extrapolated configurations, the Ekman term dissipates energy at large scales, but does not dissipate potential enstrophy. It also creates an approximately symmetric stable distribution of potential enstrophy between the two layers. The behavior of the Ekman term changes fundamentally at small scales. Under the standard formulation, the Ekman term will unconditionally dissipate energy and also dissipate, under very minor conditions, potential enstrophy at small scales. However, under the extrapolated formulation, there exist small "negative regions", which are defined over a two-dimensional phase space, capturing the distribution of energy per wavenumber between baroclinic energy and barotropic energy, and the distribution of potential enstrophy per wavenumber between the upper layer and the lower layer, where the Ekman term may inject energy or potential enstrophy.
