A Mathematical Perspective: Data for the early outpatient treatment of COVID-19 This is an abbreviated short presentation summarizing my statistical analysis of case series that provide evidence in support of both Zelenko's protocol and McCullough's protocol for early outpatient trestment of COVID-19. The presentation should be accessible to undergraduate students, and is more conceptual and less mathematical. We propose a statistical framework for analyzing the case series of patients treated with such new protocols, that enables a comparison with our prior knowledge of expected outcomes, in the absence of treatment. The goal of the proposed methodology is not to provide a precise measurement of treatment efficacy, but to establish the existence of treatment efficacy, in order to facilitate the binary decision of whether the treatment protocol should be adopted on an emergency basis. The methodology consists of a frequentist component that compares a treatment group against the probability of an adverse outcome in the absence of treatment, and calculates an efficacy threshold that has to be exceeded by this probability, in order to control the corresponding p-value and reject the null hypothesis. The efficacy threshold is further adjusted with a Bayesian technique, in order to also control the false positive rate. A random selection bias threshold is then calculated from the efficacy threshold to control for random selection bias. Exceeding the efficacy threshold establishes the existence of treatment efficacy by the preponderance of evidence, and exceeding the more demanding random selection bias threshold establishes the existence of treatment efficacy by the clear and convincing evidentiary standard. The combined techniques are applied to case series of high-risk COVID-19 outpatients that were treated using the early Zelenko protocol and the more enhanced McCullough protocol

Dr. Vladimir Zev Zelenko proposed an early outpatient treatment for the SARS-CoV-2 pandemic in two letters dated March 24, 2020 and April 28, 2020, that were then followed up with a research publication dated December 2020. We introduce a mathematical technique for conducting rapid statistical analysis of observational data that can be used to make emergency decisions in the midst of a crisis, on whether to deploy new proposed treatments based on repurposed drugs. The technique is limited to the special case of repurposed drugs that have an known excellent safety record, and it can be used to rapidly establish whether the proposed treatment works, while leaving open the question of how well it works. Hydroxychloroquine and Other Outpatient Treatments for Covid-19, with Critique of Epidemiologic Methods Abstract: To most academics and clinicians, the field of epidemiology seems like a no-brainer--you just get some cases and controls, run some regression models if you are a quant, and see what's different. Is there really any science there? Epidemiology is indeed a science but what is scientific about it per se is subtle. The crucial scientific aspect about epidemiology is representativeness: that the patients are generalizable to the disease as a whole, the controls are generalizable to the population of relevance, the exposure measures accurately reflect what the subjects really experienced, and the exposure association is not reflective of some other reason why an association is present. Epidemiologists agonize over these issues in terms of bias and confounding, as well as misrepresentation of studies, cherry picking and other more general corruptions of the scientific method. In this talk, I will discuss how non-scientific factors created aberration of knowledge greater than the scientific evidence itself, and show what the evidence for early treatment really does show. This is one of the best McCullough interviews I have seen so far, going over each of the four pillars of pandemic response in detail and in very clear language that is easily understandable by the layman. Dr. McCullough explains how the first pillar of contagion control could have been addressed with virucidal nasal and oral washes. He goes over the failure of public health agencies to encourage the deployment of early outpatient treatment and the failures of the hospitals with respect to inpatient treatment. He also discusses the safety issues with the genetic COVID-19 vaccines, the toxicity of the spike protein, and his new protocol for removing the spike protein from the body. I have captured two very interesting clips by Dr. William Grace affiliated with the Lennox Hill Hospital, that are still available from Facebook from the first year of the pandemic on the role of hydroxychloroquine, vitamin D, and N-acetyl-cysteine. The March 22, 2020 clip is particularly interesting because it shows that we had very good understanding of the pathophysiological mechanisms by which covid-19 damages the human body. The McCullough protocol is based on these insights. surprisingly this video clips had not been censored by Facebook at the time that I captured them.
