The simplest, biggest questions I hear:

Why are some of your teams doing great and other teams are not?

You have clear plans and a strong strategy and yet you feel like you're pushing people through treacle?

The answer is usually in the hands of your managers. Mostly - they mean well, but don't know what they're doing.

Thankfully, it's an easy fix.

The highest leverage thing you can do as a leader is to develop your management teams on the jobs they need to do to get things done faster.

Bonus - people will enjoy their jobs more because they are making progress.

Double bonus - people will stay longer because they have better managers and enjoy their jobs.
