The Best of Dr Sher - Egg Whisperer - Best Ovarian Stimulation Protocols for IVF
Dr. Geoffrey Sher is back on The Egg Whisperer Show Podcast, and this time, it's his 4th appearance in a week-long series! Join us as we dive deep into the world of IVF and discuss Dr. Sher's unique approach to the best ovarian stimulation protocols.
Dr. Sher spills the beans on the secrets to optimizing egg and embryo quality for the best shot at IVF pregnancy. No more one-size-fits-all – it's time to tailor the approach to each patient.
Dr. Sher breaks down the science: the two compartments of the ovary, the delicate dance of hormones, and why protocol customization is crucial. Too much or too little testosterone? It's all about finding that sweet spot for ideal follicle development.
A key takeaway? The "LH/hCG trigger" is a critical point in the IVF process, and protocol individualization is the secret sauce for success, especially for women with diminished ovarian reserve.
We also explore how Dr. Sher utilizes birth control pills, GnRH agonists, and antagonist switches to create the perfect recipe for success. Plus, he shares insights on estrogen priming for those with low ovarian reserve.
And here's the twist – Dr. Sher's groundbreaking approach extends to offering EZ-IVF, where low doses of gonadotropins are used every other day.

The Best of Dr Sher - Egg Whisperer Show - What is IUI? History of IUI and How it Works
We're back with our second episode in "The Best of Dr. Sher on The Egg Whisperer Show" podcast featuring our Associate Medical Director, Dr. Geoffrey Sher. Dr. Sher is the literal pioneer of Intrauterine Insemination (IUI).
In 1982, Dr. Sher and his colleagues pioneered the way by being the FIRST to wash and centrifuge sperm before injecting it into the uterine cavity with a catheter. Talk about groundbreaking!
🥚 Curious about IUI and why it might be recommended for some patients? We've got you covered! Dr. Sher and Dr. Aimee talk all about how IUI works, the differences between IUI and IVF, and when to choose one over the other.
👩⚕️ Not always discussed: they talk about how IUI doesn't improve pregnancy rates per treatment cycle for regular ovulators. The truth is that for women ovulating regularly, the success rate is around 10% with IUI, maybe a maximum of 15% based on your age.
👩⚕️Women with irregular ovulation or conditions like PCOS have better success rates with IUI than women with regular ovulation and normal ovarian reserve when IUI is combined with Controlled Ovarian Stimulation (COS) using gonadotropins.
Tune in and learn on The Egg Whisperer Podcast as we unravel the mysteries of IUI with the one and only Dr. Geoffrey Sher! 🎧
Plus, keep tuning in all week as we countdown to the most popular interview with Dr. Sher (and the most downloaded episode ever on The Egg Whisperer Show), which airs on Friday 12/1.

The Best of Dr Sher - Egg Whisperer Show - Treating Endometriosis-Related Infertility
We're excited to present the first podcast in our "Best of Dr. Sher on The Egg Whisperer Show" series that will be running all week.
Today's topic: "Treating Endometriosis-Related Infertility." Join us as we delve into the fascinating world of endometriosis with our Associate Medical Director, Dr. Sher. Here are some eye-opening facts he's sharing:
1️⃣ Endometriosis affects about 10% of all women, presenting a spectrum of symptoms or none at all.
2️⃣ It's estimated that 25%-40% of female infertility is linked to endometriosis in some way.
3️⃣ Endometriosis is a lifelong condition, although it can be medically or surgically managed.
4️⃣ Removal of endometriomas from an ovary may enhance chances of getting pregnant.
Dr. Sher emphasizes, "It’s very important that doctors refer patients with early endometriosis who don’t have children or who want to have more children later on to people like us for Fertility Preservation (FP-egg banking) to extract their eggs and freeze them for future dispensation. If you do nothing about it in a timely manner, by the time the woman reaches 35y her egg quality might start to decline rapidly and the likelihood of DOR (Diminished ovarian reserve) increases.
While no one wants to waste time and money, there is also the matter of emotional stress. People often ignore how emotionally debilitating infertility can be. To put someone on a course which is not going to benefit their end point objective, in my opinion, is not good medicine. So, I agree with you, and I applaud that statement. All doctors who detect endometriosis in young women should at least talk to them about infertility or refer them to people like us where we can talk to them and explain these realities so that they can prepare themselves."
Tune in to gain valuable insights on Endometriosis and fertility.
And come back tomorrow for more of Dr. Sher’s wisdom, as we count down to what is the MOST downloaded episode ever of The Egg Whisperer Show.
