Popping a GREAT Pilar Cyst
OK, this is a GREAT pilar cyst excision on the scalp of one of my longstanding patients who is so dear to my heart!
She has had this bump on her scalp for many years now, and would love it removed, but wasn't sure what was involved in this process, so she really left it along for many years.
She was happy to learn that it is a pretty simple process, and is just a procedure under local anesthesia. I was quite surprised and excited to see that this pilar cyst just popped out whole with just a little pressure and a squeeze!
I was happy to do this procedure for her, she was so happy to get it removed, and I'm sure all of you will be thrilled to watch this cyst extraction!

Incision & Drainage of Inflamed Cyst behind left ear
This patient was so adorable, because she is down at her family home, visiting from college in another part of the state, and she is accompanied by her mother. I asked her mom and the patient if we could film this procedure and I explained why. Her mom had never heard of Dr Pimple Popper, but she is a popaholic, for sure! Once I showed her @drpimplepopper on instagram, she didn't pay any attention to us or the procedure we were doing on her daughter. Of course, she loves her daughter, but her daughter was very comfortable during our incision and drainage procedure here. Her mom was entranced with my instagram page and she couldn't wait to show her friends. It was just so cute!
This patient has an epidermoid cyst behind her left ear, which has recently become inflamed and “infected”. I put “infected” in quotes, because technically this is not an infection: When an epidermoid cyst ruptures, it creates a vigorous foreign body inflammatory response - this skin becomes warm, tender, painful, and swells, simulating an abscess. Incision and Drainage (I & D), will confirm the diagnosis of inflamed cyst, when the cheesy, sometimes odiferous material is evacuated, and this process often leads to rapid resolution of symptoms. These episodes are often misdiagnosed as “infection” of the cyst, but cultures are usually negative and antibiotic treatments is not required. Intralesional steroids can hasten the resolution of symptoms as well.
All this being said, we often prescribe antibiotics- it’s hard not to when you see something so red and inflamed and painful.. it seems almost cruel not to! Also, we as dermatologists, will just call the cyst infected, or call it an abscess. It’s more difficult to explain to patients why we would not call an area that looks angry and red and is painful non-infected. However, I felt I needed to clarify all of this in detail here, for medical education purposes!

Eruptive Vellus Hair Cysts, EVHCs, appear as multiple, maybe even 100s of 1-4 mm skin colored or hyper pigmented / bluish dome shaped papules on mid chest and upper extremities. These tend to be smaller than steatocystomas and can have a central area of umbilication (almost like a punctum). Steatocystoma may at times have vellus hairs, and EVHCs may have sebaceous glands (oil glands) in their lining, which leads some dermatologists to believe these two conditions may lie along a continuum, that they may be related. Treatment is gentle extraction. There is a high chance of recurrence if a CO2 laser is used to extract them, and topical tazarotene has also been used, but is an inferior treatment.

An interesting POP on the elbow
This was a last minute kind of "pop". This is the courier/driver for our dermatopathology slide service. We see him a few minutes on a daily basis at work. Well, a couple weeks ago, he showed me this bump that he had near his elbow, and before he had time to get really nervous about it, we set him up for a quick excision!
It was interesting too, and you will see why...
So actually, I don't have the exact diagnosis, but I believe that he had a pilomatricoma with a surrounding minor hematoma/blood clot from recent trauma. Unfortunately, when they embedded the tissue, they only embedded the blood clot and not the white "bump" that I squeezed out. He looks great, and we thank him for letting us film this procedure!

First Try with New Camera! A Cyst Excised on the Left Cheek
An epidermoid cyst (Epidermal Inclusion cyst, Infundibular cyst), is a benign growth commonly found in the skin and typically appears on the face, neck or trunk, but can occur anywhere on the body. Another name used is “sebacous cyst” but this is actually an antiquated misnomer, and is not a term used by dermatologists. They are also the most common type of cutaneous cysts. Epidermoid cysts result from the reproduction of epidermal cells within a confined space of the dermis. The pasty contents are mostly composed of macerated keratin (wet skin cells), which creates this “cheesy” consistency, and there can be a pungent odor. An epidermoid cyst may have no symptoms and are typically harmless. Usually people seek removal but they don’t like the appearance of these bumps, or the cyst has ruptured or been inflamed or “infected” in the past. Rupture is associated with sudden redness, pan, swelling, and local heat, and can lead to abscess formation. Also, a history of inflammation, often increases scar tissue in the area, makes the cyst more firmly adherent to surrounding skin, and makes it more difficult to remove. Surgical excision is curative, but the complete cyst removal including the entire cyst sac and contents need to be removed to ensure that the cyst won’t reoccur.

Cysts, Dilated Pore of Winer, Blackheads & more! | Medley
A Dilated pore of Winer is essentially a large, solitary open comedone/blackhead. Dead skin cells get trapped and help widen this pore, and plugs up the opening. The expression of this plug squeezes out the macerated, white, soggy keratin/skin cells from the deeper portion of the pore. Once the content of the dilated pore is expressed, this whole process of the dilated pore filling once again with keratin is common. They are completely benign and are usually expressed for cosmetic reasons. A comedone extractor can be used to do this.
A blackhead is also called an open comedo (single for comedone), and it is a clogged pore in the skin that is open to the air. Keratin (skin protein) and sebum (oil) combine to block the pore. They are often found on the face and trunk. but they can be found anywhere on the body. Blackheads are not clogged with dirt, but it is the exposure to air that causes oxidation turning the internal contents black’. They can be extracted using a comedone extractor. I usually use an 11 blade (a blade that comes to a sharp point) and a Shaumberg type comedone extractor.

Tough blackhead extractions on the nose, Part 1
This young lady travelled out of state to come and see me, mainly because she has a mole on her chin that she wants to have removed (you will see this in a future video). She also has numerous small comedones (whiteheads) on her nose and central cheeks and forehead.
These comedones are extremely stubborn to extract.. I would call many of them keratin filaments.. just keratin really wedged into her pores/follicles. Since this was her first visit to see me, I recommended she return again after applying RetinA (tretinoin), which is a prescription product, to the area in order to help soften up these comedones so they would be easier to extract in the future.
As you can see, her whiteheads were so stubborn! She would have extracted them herself if she had the right tools, but she could really bruise herself trying to remove these. I believe that if she were to start using tretinoin for a few weeks, future extractions will be easier and her skin will look so much smoother. She'll be back in a week and we will see if we can extract more next time!
I can't say it enough: I have the most wonderful patients, who are so sweet and so kind to allow me to film them for all you to watch, learn and enjoy!

She had two inflamed lesions recently, one was a cyst and the other was probably a Dilated Pore of Winer which got inflamed, one after the other. A big blackhead, perhaps a Dilated Pore of Winer!
This is a Dilated Pore of Winer, aka a big blackhead. These are usually a little more superficial under the skin, and can be pretty large. The keratin (skin cells) are oxidized, which give it a dark color. This man had this on his back for awhile, and came to see us because he knew that we were the pimple popping experts! ;) FYI, my left hand shakes when I use a needle to anesthetize the area on purpose. Shaking or pinching the skin when using a needle is a distraction technique. You feel less pain if you vibrate or pinch the area simultaneously.
Extracted using our DrPimplePopper comedone extractor, you can see that the sac was removed completely, and that the internal contents a are a little "sticky" and a little difficult to remove. I think we got it all!

Hidrocystomas on both eyes!
Hidrocystomas, also known as cystadenomas, sudoriferous cysts, and Moll's gland cysts, are benign cyst tumors that are derived from either the eccrine or apocrine sweat gland. They commonly effect the eyelid, and slowly grow usually on the face or scalp. They both affect males and females, are usually asymptomatic and are more common in adults. Apocrine hidrocystomas aren't affected by temperature, but the eccrine hidrocystomas grow smaller in cooler temperatures and larger in heat exposure. The exact cause is not known but it is said that the eccrine hidrocystoma is due to the obstruction of the sweat gland which causes the retention of secretions and appearance of a dilated cystic structure. The apocrine hidrocystoma is thought to be an adenoma of the coil structure of the apocrine sweat gland. These are found only on certain areas of the body. For example, eyelids, armpits, areolae, external ear and genital region. Compared to the eccrine sweat glands, the ductal openings of the apocrine sweat glands are more closely associated to the eyelashes. The Eccrine sweat glands are found distributed throughout the whole body.

This Cyst is For His Tia..
A Pilar Cyst, aka a trichilemmal cyst, an isthmus-catagen cyst, or a “wen”, looks identical on examination to the more common epidermoid cyst, except that 90% of them occur on the scalp. They tend to be more mobile and firmer than epidermoid cysts, and are derived from a different part of the hair follicle/skin. The internal content tend to be more “homogenous”/ smooth (looks different under the microscope), but it is also a benign growth. It is not uncommon to have multiple pilar cysts on the scalp and to have other family members to have these cysts (tend to run in families). Treatment is for cosmetic reasons usually (people don’t like the appearance of them), and excision with removal of the cyst and it’s contents is curative.

A Surprise Steatocystoma!
Steatocystoma Multiplex appear as multiple, uniform, yellow, cystic papules usually 2-6 mm diameter, located especially on upper anterior trunk, upper arms, axillae, and thighs. Typically no central punctum or umbilication is seen. Majority of cases present with dermal lesions, but multiple subcutaneous masses looking like multiple lipomas can be present. Bumps usually appear in adolescence or early adulthood, probably because sebaceous activity is at its peak. Sometimes larger steatocystomas are prone to rupture and suppuration and can cause scarring and pain.
Steatocytomas typically contain a syrup-like, yellowish , odorless, oily material. If they are inflamed or infected by bacteria, they can develop a foul odor and can be a definite source of social isolation.
This is probably the closest thing to a “sebaceous cyst”, a name that many people including non dermatologist physicians may use to describe an epidermoid cyst which we see more commonly. Steatocystoma may at times have vellus hairs, and eruptive vellus hair cysts may have sebaceous glands (oil glands) in their lining, which leads some dermatologists to believe these two conditions may lie along a continuum, that they may be related.
Treatment is removal with small incisions and extraction, with care on treatment on the trunk because of the increased chance of poor cosmetic result/scarring.

She is an example of a beautiful woman who gets multiple cysts on her back, and although you can't really see them, she can FEEL them and they make her feel very self conscious. She is very happy to have them removed and was so kind to let us film the procedure. In fact, she says that the procedure is very cathartic for her... she is very relaxed and relieved these bumps are being removed.

A Cyst, A Blackhead, & a Dilated Pore of Winer!!
An epidermoid cyst (Epidermal Inclusion cyst, Infundibular cyst), is a benign growth commonly found in the skin and typically appears on the face, neck or trunk, but can occur anywhere on the body. Another name used is “sebacous cyst” but this is actually an antiquated misnomer, and is not a term used by dermatologists. They are also the most common type of cutaneous cysts. Epidermoid cysts result from the reproduction of epidermal cells within a confined space of the dermis. The pasty contents are mostly composed of macerated keratin (wet skin cells), which creates this “cheesy” consistency, and there can be a pungent odor. An epidermoid cyst may have no symptoms and are typically harmless. Usually people seek removal because they don’t like the appearance of these bumps, or the cyst has ruptured or been inflamed or “infected” in the past. Rupture is associated with sudden redness, pain, swelling, and local heat, and can lead to abscess formation. Also, a history of inflammation, often increases scar tissue in the area, makes the cyst more firmly adherent to surrounding skin, and makes it more difficult to remove. Surgical excision is curative, but the complete cyst removal including the entire cyst sac and contents need to be removed to ensure that the cyst won’t reoccur.
A blackhead is also called an open comedo (single for comedone), and it is a clogged pore in the skin that is open to the air. Keratin (skin protein) and sebum (oil) combine to block the pore. They are often found on the face and trunk. but they can be found anywhere on the body. Blackheads are not clogged with dirt, but it is the exposure to air that causes oxidation turning the internal contents black. They can be extracted using a comedone extractor. I usually use an 11 blade (a blade that comes to a sharp point) and a Shaumberg type comedone extractor.
A Dilated pore of Winer is essentially a large, solitary open comedone/blackhead. Dead skin cells get trapped and help widen this pore, and plugs up the opening. The expression of this plug squeezes out the macerated, white, soggy keratin/skin cells from the deeper portion of the pore. Once the content of the dilated pore is expressed, this whole process of the dilated pore filling once again with keratin is common. They are completely benign and are usually expressed for cosmetic reasons. A comedone extractor can be used to do this.

Small cyst on right cheek excised with suspension stitch
I see this patient mainly for cosmetic issues, and the last visit with her I noticed this bump on her face that she has been dealing with for a while. I think she will be happy to have it removed. I used a suspension suture here, as I have done in some patients in the past.
