Denying Your Health Care Is Big Business in America | NYT Opinion
Should your insurance company be allowed to stop you from getting a treatment — even if your doctor says it’s necessary?
Doctors are often required to get insurance permission before providing medical care. This process is called prior authorization and it can be used by profit-seeking insurance companies to create intentional barriers between patients and the health care they need.
At best, it’s just a minor bureaucratic headache. At worst, people have died.
Prior authorization has been around for decades, but doctors say its use has increased in recent years and now rank it as one of the top issues in health care.
To produce the Opinion Video above, we spoke to more than 50 doctors and patients. They shared horror stories about a seemingly trivial process that inflicts enormous pain, on a daily basis. The video also explains how a process that is supposed to save money actually inflates U.S. health care costs while enriching insurance companies.
Prior authorization has come under intense scrutiny in Congress in the past few years, but bipartisan proposals have repeatedly stalled. Under public pressure, some insurance companies — like United Healthcare and Cigna — have said they would reduce the use of prior authorization. And in January, the Biden administration finalized a plan to put limited guardrails around this practice. But doctors say that these efforts only scratch the surface and should go further.
This issue is ultimately about the role of insurance companies in American health care: Should they have more power than your doctor to decide what’s medically best for you?

Every pregnancy is unique, and not all mothers will experience the same symptoms or progress in the same way.

In preparation for birth, The baby may experience position changes, such as shifting from side to side or rotating within the womb. Most babies settle into a head-down position in the mother's pelvis, with their head closer to the birth canal. This relieves pressure on the diaphragm and makes breathing easier for the mother. It may also increase pressure on the bladder, leading to more frequent urination. This position, known as "cephalic presentation," is considered ideal for vaginal delivery. However, some babies may remain in a breech or feet-first position.
Throughout the final weeks of pregnancy, The baby's lungs continue to mature And produce a substance that helps keep the air sacs open and facilitates efficient oxygen exchange after birth. The baby may engage in practice breathing movements In the last weeks. These movements involve rhythmic contractions of the diaphragm and chest muscles, aiding in lung development and preparation for breathing air after birth.
In anticipation of labor and impending parenthood, the mother can bring about a range of emotions. She may feel excited, anxious, nervous, or a combination of these emotions.
Many mothers experience a surge of energy and a strong desire to prepare for the baby's arrival. This is commonly referred to as the nesting instinct. She may have an intense urge to clean, organize, and set up for the baby.
Braxton Hicks contractions can occur as early as the second trimester but become more noticeable in the last weeks of pregnancy. They are often described as practice contractions and help prepare the uterus for labor.
As labor approaches true labor contraction occurs. True labor contractions have a regular pattern and become increasingly frequent, longer, and more intense over time. They lead to gradual dilation of the cervix.
The amniotic sac rupture, or water breaking, can occur at different times during labor. It may rupture on its own without any intervention, or if the amniotic sac hasn't ruptured naturally, the healthcare provider may intentionally rupture the amniotic sac using a sterile instrument called an amniotomy hook

How does your body turn food into poop?
Mechanical digestion begins in the mouth as the food is chewed.
Chemical digestion begins in the mouth when food mixes with saliva. Saliva contains an enzyme (amylase)
that begins the breakdown of carbohydrates.
The epiglottis is a flexible flap at the end of the larynx in the throat. It acts as a switch between the larynx and the esophagus to permit air to enter the airway to the lungs and
food to pass into the esophagus. it is a muscular tube connecting the throat with the stomach. The esophagus is about 8 inches long.
The esophagus muscle acts with peristaltic action to move swallowed food down to the stomach. The action of peristalsis looks like an ocean wave moving through the muscle
The food then enters the stomach, which is a rounded, hollow j shape organ Located between the esophagus and the duodenum. the stomach has three mechanical tasks to
do. First, the stomach must store the swallowed food and liquid. This requires the
muscle of the upper part of the stomach to relax and accept large volumes of swallowed
material., The inner layer of the stomach is full of wrinkles known as rugae. Rugae both
allow the stomach to stretch in order to accommodate large meals and help to grip and
move food during digestion.
The second job is to mix up the food, liquid, with digestive juice produced by the
stomach. The lower part of the stomach mixes these materials as a blender by its
muscle action. and breaks down the food into tiny particles in 3 millimeters
finally, the stomach empties this acidy mash slowly into the small intestine.
The small intestine or small bowel is an organ in the gastrointestinal tract where most of
the end absorption of nutrients and minerals from food takes place. It lies between the
stomach and large intestine and receives bile and pancreatic juice through the
pancreatic duct to aid in digestion.
The Duodenum, the first part of the small intestine, receives partially digested
food from the stomach and begins the absorption of nutrients. The duodenum is
the shortest segment of the intestine and is about 23 to 28 cm (9 to 11 inches)
long. It is roughly horseshoe-shaped
The pancreatic juices and bile that are released into the duodenum, help the body to
digest fats, carbohydrates, and proteins.
The food is in instetin now and has a very long way to go!
As a person grows the small intestine increases 20 times in length from about 200 cm in a
newborn to almost 6 m in an adult.
The inner walls of the small intestine show mucosal folds. These are called the plicae
circulares. The plicae are more numerous in the early and reduce in numbers in the later
part and are completely absent.
Digestion is important because your body needs nutrients from food and drink to
work properly and stay
healthy. Proteins, fats, carbohydrates, vitamins, minerals, and water are
nutrients. Your digestive system breaks nutrients into parts small enough for
your body to absorb and use for energy, growth, and cell repair.
•Proteins break into amino acids
•Fats break into fatty acids and glycerol
•Carbohydrates break into simple sugars
In the small bowel, the food particles get even smaller. Here is where all the
important vitamins and nutrients in food move through the blood vessels that are
in the lining of the small bowel. The blood takes the nutrients to other organs in
the body. The nutrients are used to help repair cells and tissue.
What is left over, which is mostly liquid, then moves into the colon. The water
is absorbed in the colon. Bacteria in the colon break down the remaining
material. Then the colon moves the leftover material into the rectum.
the rectum is like a storage-holder for this waste. Muscles in the rectum move the
waste, called stool, out of the body through the anus.
