All are required to cover two drugs from every category and substantially all drugs from the six protected categories, which include cancer drugs, autoimmune drugs and so on. Most Part D plans are currently robust and cover thousands of drugs, much more than the minimum. That may change, I’ll get back to that. The Part D bundled with the Advantage plan does not comingle with the Advantage plan. It has a separate deductible, a separate network and is not subject the maximum out-of-pocket of the Advantage plan. There is nothing in the Inflation Reduction Act that will impact Stand Alone drug plans, but NOT drug plans bundled with Medicare Advantage Plans. They are the same, it’s just how you purchase them. Of course, those who have seen my videos on Part D already know that. CMS could eliminate all Medicare Advantage plans today and overnight it would reduce the financial burden on the Government and improve healthcare for the consumer. And you want to argue that Private is better? When private has access to a cornucopia of government funds, the result is corruption. Medicare Advantage Plans are the fiscal and regulatory nightmare the republicans fear they would have found in Original Medicare. But they get so caught up in this idea that everything private is better than anything government they don’t stop to look at reality. They don’t stop to question their own assumptions. Reality is that our healthcare is system is neither government controlled nor private free market capitalism. It is a system of corporate welfare and backdoor deals. It’s one where no corporate crime is penalized, and political donations will buy off a vote to push an agenda based on profit. With original / traditional Medicare and a supplement you can put all that aside. You can escape it. You are shielded from the politics and in part the corruption. That’s a good choice. Medicare supplement Plan N vs Plan G. Why? Because with a Medicare supplement plan you keep the two most important features of Medicare. You can see any doctor or go to any medical facility in the US or US territory as long as they accept Original Medicare. You are not limited to a network and you do not need to get a referral to see the doctor of your choice. In addition, no insurance has a say in your healthcare. No insurance company can demand your doctor seek their preauthorization before providing you the treatment as recommended. No insurance company can deny your coverage. Your healthcare decisions are between you and your doctor. You give up both of these rights when you choose to trade in your Medicare for an Advantage Plan. Medicare supplement Plan G is what we call your Peace of Mind Plan. You have no office visit copays and no coinsurance. Your annual maximum out-of-pocket is the Part B deductible. Again, for 2023 that is $226. In 2010 when Plan N was new, Medicare released a clarification on the copays that gave specific billing codes required to charge an office copay. The office visit copay is 20% of the bill up to a maximum of $20. Those billing codes were exclusive to in-office visits to your primary care physician or a specialist and can be no more than $20. Medicare’s guidance specifically indicated there is to be no office visit copay for telehealth or urgent care visits. During the pandemic, under part of the emergency order, billing rules were bent for telehealth doctor visits and Urgent care. In both cases, Medicare quietly allowed doctors to bill these visits using the same billing code for in-office primary care visits. So, where telehealth and urgent care visits were originally excluded from the $20 copay prior to the pandemic. A number of our clients are IBM retirees who chose to work with us in years past and have asked my opinion on the new Group Advantage Plan choices. I have read the material and have seen the presentation by United Healthcare, and I believe it certainly deserves some comments and clarifications. That is what I will do with this video. If you are still using Via Benefits for a supplement, as of January 2023 you are no longer incentivized to use them, so and I am pretty sure we can lower your premiums. The supplement plans they offer are specially priced for those who have chronic health problems need a guaranteed issue plan. If you are not in such poor health that you need a guaranteed issue, you will be pleasantly surprised when you let us compete for your business. A dose of reality here: IBM retirees who went with Via Benefits years ago because they were told that is what they have to do to get the IBM subsidy, now are forced to make a change or lose the IBM subsidy. They are in overpriced supplement plans and have now lost a subsidy that at least reduced the cost somewhat. In the first line they state that the plans have combined features of a Medicare supplement, Part D and Medicare Advantage Plans. This is the first of a series of comments that led people I have talked with to believe this is some enhanced Medicare Advantage Plan. It’s not. By the way, all Medicare Advantage plans are custom designed. Some are custom designed for your area, some for a specific group, like these. The plans being shown for 2023 are Medicare Advantage Prescription Drug plans. They are nothing more than a Medicare Advantage PPO with a bundled Part D plan. I have looked at all the material provided, including the Summary of Benefits and cannot find any specific feature of a Medicare supplement within these plans. When you are shopping for a Medicare Advantage Plan, there are two features you should look at first. First look at your maximum out of pocket. This is the most you will pay for inpatient and outpatient care if you play by the insurance company rules. Second, look at your cancer coverage. I suspect it will be that you pay 20% of the cost of treating cancer up to your maximum out of pocket. That's 20% of chemotherapy, radiation therapy, and Part B cancer drugs. If you are uncomfortable with the maximum out of pocket, consider cancer protection insurance. More than twice the rate of inflation as was reported by the CPI at the time. The base Medicare Part B premium increased from $148.50 to $170.10 a month with the stroke of a pen by the department of Health and Human Services. The base premium refers to the rate for those whose modified adjusted gross income was reported as $91,000 / year of less in 2020. $182,000 for joint filers. The reason for the increase was credited to a new Alzheimer’s drug produced by Biogen called Aduhelm. Aduhelm is a new type of drug called a monoclonal antibody treatment that focuses on clearing specific plaque in the brain called Amyloid Beta. A 2006 research study indicated Amyloid beta was the plaque that caused Alzheimer’s. Biogen intended on charging $56,000 for the series of infusions they suggested as a full dosage. The Centers for Medicare & Medicaid Services (CMS) and Social Security determine your eligibility for IRMAA payments by calculating your annual Modified Adjusted Gross Income (MAGI). This is simply your Adjusted Gross Income from your tax return with some deductions added back to your income. The MAGI used is as of two years ago. In other words, CMS recalculates your MAGI on a rolling two-year lookback. CMS then informs Social Security of their determination. Some of the deductions added back to your income to determine your MAGI include: • Untaxed Social Security payments • IRA Contributions • Any passive income loss • Deductions for tuitions and fees Your MAGI is then matched with tax filing status and income levels in the table below to determine the amount due. For 2022 the Part B premium ranges from $170.10 per month (standard) to a high of $578.30 per month for those in the highest income bracket. Can I Avoid an IRMAA Surcharge with Medicare Advantage Plans? The short answer is no. IRMAA charges are based on your income and will be assessed regardless of how you get your Medicare. How To Appeal an IRMAA Surcharge If it is determined that you should pay an IRMAA surcharge, you will receive an Initial Determination notice from Social Security. The notice will include information on how to request a new initial determination. You will want to appeal the IRMAA decision if you believe the was incorrect. For example, if you had corrected or amended a previous tax return resulting in a lower reported income. Also, there are certain life-changing events that will be considered in reevaluating your IRMAA status. Social Security will consider these life changing events when re-examining your IRMAA status: • The death of spouse • Marriage • Divorce or annulment • You or your spouse stopped working or reduced the hours worked. • Loss of pension • Involuntary loss of income-producing property • Receipt of settlement payment from a former or current employer due to closure or bankruptcy How to Avoid an IRMAA Surcharge Below are a list of ways a person can avoid the IRMAA surcharge. 1. Plan ahead. Now that you know the IRMAA calculation uses your income as of two years ago, avoid actions that will influence your income up to two years prior to starting Medicare. 2. Be careful with Roth Conversions. You can accidentally push your Part B premium from $170 a month to nearly $600 a month. 3. Required Minimum Distributions (RMDs) If an RMD will push you into a higher bracket you can use charitable contributions to lower your income. If you don’t need the income from your RMD, your philanthropy can benefit you as well as your favorite charity. 4. Consider the tax advantaged income of annuities and life insurance. The short answer is that Medicare does not cover most dental care. This includes cleanings, fillings, dentures, extractions and so on. It also does not cover eye exams for eyeglasses or contact lenses. However, Medicare Part A, inpatient coverage, may cover certain dental services that are related to your inpatient hospital stay. For example, it is not uncommon for your doctor to require an oral exam and treatment before an organ transplant or heart surgery. This may also include emergency or complex dental procedures related to facial cancer, reconstructive surgery and so on. That is covered by Medicare. Regarding vision, keep in mind that Medicare will cover eye diseases or conditions like cataracts and glaucoma. But it does not cover corrective vision. Do not trade your Medicare in for a Medicare Advantage Plan, do not trade your Tricare or ChampVA in for a Medicare supplement, and do not get a stand-alone Medicare Part D plan. I believe you cannot improve on your healthcare by messing with your TriCare. If you replace your original Medicare with an Advantage plan you are actually reducing your insurance coverage and increasing your financial risk. That’s the short answer. That’s my firm opinion. Tricare For Life is Like a Medicare Supplement For Tricare for Life to be active, you must also have original Medicare. That is Medicare Part A (inpatient coverage) and Medicare Part B (outpatient coverage). Original Medicare becomes your Primary insurer and with Tricare being secondary, just like a Medicare supplement plan. Tricare for life is premium free. You earned it. The part of this that is most important and the reason I made this video is that Original Medicare, that red, white and blue Medicare card is your primary insurance. Why is it important that original Medicare remain your primary insurance? Because you get to keep the benefits of original Medicare. There are two benefits of original Medicare that are critical to your healthcare. First, with original Medicare being your primary insurance, you are not limited to any network. You can see any doctor or visit any medical facility in the country as long they accept original Medicare. That’s pretty much every doctor and every medical facility in the US. or US territory. There are very few exceptions. Second, no insurance company can interfere with your care. Your medical decisions are between you and your doctor. The goal of both Medicare and TriCare is to cover everything that is medically necessary. Your doctor does not need to get prior approval from the insurance company. You do not need to get a referral from your doctor if you wish to see a specialist. No insurance company can stall or prevent your medical treatment based on their internal rules. All medical decisions are between you and your doctor. When you have Original Medicare, Tricare for Life and Medicare work hand in glove so that your annual maximum out-of-pocket financial risk is limited to near zero. In fact, the only expense for inpatient or outpatient services would be if TriCare approves a treatment that Medicare does not. I have never seen that, but I guess it’s possible. In that case TriCare will cover the service with a small potential out-of-pocket from you. Please see your TriCare benefit details online for specifics. I have a link to the government site below. As I mentioned, TriCare for life is like a Medicare supplement Plan on steroids. In addition to the standard Medicare supplement benefits, your TriCare includes prescription drug coverage. You do not want to get a separate Medicare Part D plan. In fact, doing so can cause harm to your TriCare, especially ChampVA. By the way, did you know that the drug coverage you have through TriCare is not subject to the expensive coverage gap that is a feature of Medicare Prescription drug coverage? You have no coverage gap, no donut hole like those with Part D have to deal with. Tricare For Life & International Coverage One of the enhanced benefits I like about TriCare is that it offers significant healthcare coverage overseas. Medicare only operates in the US and US territories. Some Medicare supplement plans offer limited emergency coverage if overseas.
