In compliance with Salt Lake City Ordinance 200.10.200, the Salt Lake City Police Department released body-worn camera footage of the officer-involved-critical incident (OICI) that occurred on November 08, 2022 at a residence near 1700 South and 900 East in Salt Lake City, Utah.
Statements in this release are based on a preliminary review of limited information available at the time of this writing. A thorough investigation is being conducted by an outside agency, pursuant to the OICI protocol required by state law. Additionally, the SLCPD Internal Affairs Unit will conduct a separate investigation.
The videos released today captured the OICI from the body-worn cameras of the two SLCPD officers on scene during the OICI, as defined by city ordinance.
Given the medical and residential privacy interests, the body-worn camera footage has been redacted in an effort to balance the public interest in the officer-involved critical incident with the privacy interests at stake. Redactions have been limited to faces of individuals other than police officers and 37-year-old George Gulla and audio capturing the conversations between those individuals and Mr. Gulla. Officers were performing a standby assist and did not engage with Mr. Gulla until the critical incident displayed in the video. The video begins once all parties enter Mr. Gulla’s residence and ends when the officers begin providing medical assistance.

“This incident is reflective of how a situation can turn dangerous with no warning. The collaboration our police department has with our own social workers and mental health professionals as well as the mental health service providers in our community is critical to more fully addressing the needs of Salt Lake City, and I am proud of that work. DISPATCHED DETAILS:
This investigation started on Tuesday November 08, 2022, at 11:32 a.m. when SLC911 received a call from a person with the Huntsman Mental Health Institute’s Mobile Crisis Outreach Team (MCOT).
The caller requested SLCPD to assist MCOT as they made contacted with George Gulla.
SLC911 dispatched the call at 11:35 a.m. and two officers went en-route at that time.
The first SLCPD officer arrived at 11:40 a.m.
The second SLCPD officer arrived at 11:43 a.m.
OFFICER ACTIONS:
Upon arrival, one of the MCOT employees contacted one of the SLCPD officers and discussed Mr. Gulla’s recent drug use, prior police interaction and MCOT’s safety concerns.
The two SLCPD officers accompanied two employees from MCOT and a family member of Mr. Gulla’s to the garage of the private residence. All parties entered the garage after Mr. Gulla’s family member opened the garage entrance door.
While inside the garage, Mr. Gulla’s family member and the mental health professionals spoke with Mr. Gulla.
The two SLCPD officers who were performing a standby assist remained inside the garage and maintained observation. During the conversation with MCOT and Mr. Gulla’s family member, he became agitated. The two SLCPD officers did not engage directly with Mr. Gulla until the officer-involved-critical incident.
From the officers’ body-worn camera, at 11:53 a.m., it appears Mr. Gulla, while sitting on a mattress, makes a sudden lunge toward the foot of the bed. One of the mental health professionals reacts to Mr. Gulla’s actions, turning and running away. As this occurs, both officers fired multiple rounds, striking Mr. Gulla.
The other mental health professional hid behind one of the officers.
After the shooting, officers started giving Mr. Gulla commands.
Once taken into custody, without further incident, officers provided medical aid to Mr. Gulla.
A weapon was recovered from Mr. Gulla’s bed.
Mr. Gulla remains at a local hospital and is expected to survive. Criminal charges are pending.
No further information is available for release. SLCPD is not available for interviews because of the ongoing investigation.

Suspect arrested, out on bond for unrelated offense after allegedly leading Vidor police on 'one of the most dangerous pursuits'.
A suspect that allegedly led police on what they believe could've been "one of the most dangerous pursuits" they have ever seen was arrested, released on bond on an unrelated offense.
On Thursday, September 22, a thief was caught red-handed after a police chase, but managed to get away.
When news of this incident first aired, the Vidor Police Department was not aware that 33-year-old Bryan Anthony Barba of Buna had been arrested in Jasper County on September 23 for criminal mischief, a third degree felony.
Chief Carroll believes it was by the grace of God that no one was injured in the pursuit. He believes Barba left thousands of dollars in property damage in his wake.
"You can see it took out mailboxes,” Chief Carroll said. “It took out a brick-and-mortar mailbox. The damage that it could have done to a human being, or another vehicle or motorcyclists would have been tragic."
This felony charge came after a report involving damage to an oil well site on County Road 707 in Buna on September 15, according to a news release from Jasper County Sheriff's Office.
He was later released on a $25,000 bond set by Pct. 4 Justice of the Peace Gina Cleveland.
The following week on September 22 around 10 a.m., a resident on Doty Road called area authorities and stated a man was stealing AT&T cables. The concerned citizen followed the man’s white Chevy pickup truck until police arrived.
Police found Barba, who they say led them on a 30-minute, high-speed chase with 100 feet of cables trailing behind his pickup truck.
Vidor police was assisted by Jasper County deputies and the Texas Department of Public Safety, according to the release.
The pickup reached speeds of more than 100 mph during the chase down Highway 105 and onto County Road 707 in Buna.
Deputies found the vehicle at Barba's home. The vehicle was then towed by Vidor police.
Vidor Police Chief Rod Carroll told news sources, "It's probably one of the most dangerous pursuits I've seen just because of what was trailing the vehicle,”
Police say Barba weaved through backroads and dirt roads, mailboxes were destroyed, trash bins were knocked over and the suspect left a cloud of debris behind him.
Vidor Police followed Barba from Orange County through Vidor, with the chase ending in Jasper at his house. It was there that Barba left his truck and ran away.
After searching the area, police found the suspect’s pickup truck with the cables still attached. Inside the man's home, police said they found more stolen cables.
"I'm not sure how much was found behind the residence," Chief Carroll said. "This has been an ongoing problem in the rural county, with individuals stealing this cable, melting it down and then selling it at the recycling locations."
Usually during a chase, police would use spike strips to stop a vehicle. Chief Carroll said his officers opted out because it was just too dangerous.
“An officer can get killed very easily,” Chief Carroll said. "The patrol unit itself was damaged. The windshield was shattered from the debris field that was sent up from it."

Baltimore police released a body-worn camera video Tuesday from the officer who shot and killed a man who was threatening a woman with a knife Sunday.
The video shows the officer, identified as Zachary Rutherford, pulling up to the scene in West Baltimore, where he sees Tyree Moorehead on top of a woman and holding a large butcher knife at her head.
Rutherford screams "drop the knife" but Moorehead continues to straddle the woman on the pavement. Rutherford then fires his weapon multiple times at close range, the video shows.
As Rutherford fires his weapons, the woman rolls away to safety. She was not shot or stabbed, police said.

Bodycam footage from the Atlanta Police Department shows officers confront and restrain a passenger who brought boxcutters on a plane outbound from CVG.
The Frontier Airlines flight was en route to Tampa, Florida, but made an emergency landing in Atlanta around 9:30 p.m. last Friday.
The passenger is William Allen Liebisch, according to a criminal complaint filed in the U.S. District Court for the Northern District of Georgia.
The complaint claims Liebisch assaulted or intimidated a flight crew member or attendant.
As shown in the bodycam footage, Atlanta police officers wait for most of the other passengers to deboard before making entry around the 6:40 mark of the video. The complaint notes two male passengers remains in their seats.
At that point, the officers take out their tasers and enter the plane cabin, where the man appears to be out of his seat and in the aisle.
The complaint notes “as the final passengers were walking off the plane, the flight attendants alerted law enforcement that Liebisch was running towards the front of the aircraft. Liebisch was then tackled from behind by one male passenger.”
One of the officers tells him, “Sir, hold on. Stop. Stop!”
After the man is tackled down on the aisle floor, the officers pat down his jacket and inquire after the boxcutter.
The officer wearing the bodycam says, “I told you, you’d better cooperate, or you’re gonna get tasered.”
The man raises his hands cooperatively. Then the officers tell him to stand up and help him to his feet. They bind his hands.
The officers tell the man to relax. He answers, “You relax, man, and put them [****]ing handcuffs on right man. I didn’t even resist. This is bananas, man. What the [****] is wrong with you?”
The complaint notes officers discovered another boxcutter during a post-arrest inventory search of his belongings.
No one was injured, according to the airline.
The man passed through the TSA checkpoint at CVG with two backpacks and additional loose items, according to a TSA spokesperson.
The items went through the CT screening, which creates a 3D image that can be rotated 360 degrees for a thorough analysis, per the spokesperson.
The boxcutters were not identified by the CT operator, who did not fully use the CT capabilities, the spokesperson said. However, the passenger’s property was identified for a further search, and through that search one boxcutter was discovered.
“The visible blades were removed from the box cutter and provided back to the passenger. This is contrary to standard operating procedure which requires these items to be placed in checked bags or voluntarily abandoned. The backpack containing the other box cutter, and the remainder of the traveler’s property, was screened for explosives, but the box cutter was not discovered,” the spokesperson said.
The TSA employees involved in the incident have been placed in remedial training on CT image review and physical search procedures.
Atlanta PD’s investigation is ongoing.

Both videos end as the subject falls to the ground. The video is stopped pursuant to City Ordinance 2.10.200 (D) (3) which states “some images from a body-worn camera might be offensive or violate an individual’s privacy and should not be disclosed…These offensive/violative images include…showing the moment an individual deceases or the individual’s body the moment after deceasing…”
OFFICER ACTIONS:
There are only two body-worn camera videos that captured the actual officer-involved-critical incident. The BWC footage for both officers starts as they are pursuing the subject. The pursuit ended in an industrial area near 220 South Orange Street after the subject crashed the stolen vehicle, rendering it inoperable after becoming high-centered. After crashing, the subject remained in the vehicle. The involved officers, as they exited their vehicle, immediately began giving verbal commands for the subject to “show me your hands” and “drop the gun.” Instantaneously, shots, in rapid succession, were exchanged between the subject and the two officers. Upon exiting the driver side of the police car, one officer took cover behind the police car while engaged with the subject. Upon exiting the front passenger side of the police car, the other officer eventually took cover behind the police car while engaged with the subject. The subject exited the stolen vehicle from the front passenger side. There is a brief pause in the volley of gunshots, which is when one of the officers sees the subject running from the front passenger door of the vehicle and past the trunk in the direction of the officers. Both officers briefly continue to discharge their service weapons as the subject falls and briefly continue shooting as he is on the ground with the gun in close reach. A tactical approach, with responding and the involved officers, is formulated to approach the subject on the ground. After clearing the gun from the subject’s immediate reach, emergency life-saving efforts are conducted. Life-saving efforts continued for approximately 15-20 minutes with officers and paramedics.

All videos end when the officers start rendering first aid.
DISPATCHED DETAILS:
At 3:35 a.m. an SLCPD officer, working a secondary employment shift, notified SLC911 via police radio that he had arrested a woman, later identified as Ms. Mohn.
OFFICER ACTIONS:
During the encounter with Ms. Mohn, a struggle occurred between Ms. Mohn and SLCPD officers, which resulted in officers using arrest control techniques. While being restrained, officers placed Ms. Mohn on her stomach.
Officers gave Ms. Mohn multiple commands to stop resisting.
Both handcuffs and ankle restraints were used by SLCPD officers.
Ms. Mohn became unresponsive.
Officers, as depicted on body-worn camera video, started performing medical assistance.
Gold Cross transported Ms. Mohn to Salt Lake Regional Hospital in critical condition.
SLCPD later learned, on February 9, 2022, that on January 28, 2022, hospital staff moved Ms. Mohn into the intensive care unit at Salt Lake Regional Hospital.
She died on January 30, 2022.
On July 28, 2022, the Utah Office of the Medical Examiner submitted a report to the Salt Lake City Police Department which concluded the death of Ms. Mohn to be a homicide.
Salt Lake City Police started the OICI protocol on July 29, 2022, after reviewing the medical examiner’s report.
NOTE:
The family of Ms. Mohn is requesting privacy at this time and does not wish to be contacted by the media.
No further information on this case is being released. Neither the PIO nor the On-Duty Watch Commander are available for interviews.

In the early morning hours of Nov. 6, Razorback football players Myles Slusher and Anthony Brown were arrested by Fayetteville police near Dickson Street.
According to the Fayetteville Police Department's (FPD) timeline of events, officers were patrolling the area of 336 N. West Ave. on foot while trying to get people out of the street after bars and businesses had closed.
FPD says one of the first people they told to get out of the road was 19-year-old Anthony Brown. At first, they claim Brown agreed and got out of the street but then went back, intentionally standing in the roadway a second time.
Police claim an officer grabbed Brown’s sleeve and walked with him to the sideway. That’s when police say Brown turned and pushed a Fayetteville Police Officer. They claim Brown was then taken to the ground and put into custody.
During the arrest of Brown, police say 20-year-old Myles Slusher was allegedly grabbing an officer, trying to pull him away from arresting Brown.
According to police, multiple officers were able to separate Slusher from the altercation and put him into handcuffs.
Police claim they asked Slusher multiple times for his name and he refused to give him any information. As the transport unit arrived on the scene, police say Slusher eventually gave him his name and date of birth.
Brown and Slusher were booked in the Washington County Detention Center.
Adam Rose is representing Slusher and says the arrest was an "overreaction" to the situation at hand.
"I don't think this wasn't a case where an arrest really should have been made. If the officers really felt strongly about I think a citation was more appropriate," Rose said. "I think that it was quite an overreaction. And I think if you watch the video... I think it was quite a large reaction from the officers."
"There's a lot of people and if you watch the body cameras, a lot of people are around and they just so happened to get upset with Mr. Brown and Mr. Slusher over not following instruction as to where he's stepping," Rose said.
Rose says he is only representing Slusher and his upcoming court date has been waived. He also said they are in talks with the prosecuting attorney to get this "taken care of" quickly.
"He's a bright young guy, smart guy. He's a good kid. And I don't foresee this really affecting him too much in the future," Rose said. "I spoke with his parents and his mother specifically. And they feel very passionately that they want the truth to get out there that their son's not a bad guy, that this was not a horrific incident."
Rose believes there is a misconception about the severity of Slusher's behavior, especially after watching the bodycam video.
"And the severity of it is he inquired as the officer why his friend was being arrested, he did touch the officer on the shoulder, didn't assault the officer, he's not charged with assaulting an officer, not with battery of an officer, nothing like that," Rose said. "And so I just want the public to know that he respected the officers. He was just simply asking why his friend was being arrested, and frankly, I believe that he should have been given that information."
Rose says whether or not they will file civil litigation against the FPD is pending right now.
"I think the next step for Mr. Slusher is getting this taken care of, getting this behind them, not letting this affect him in the future and just moving forward and focusing on what he does best, which is playing football," he said.
Both Slusher and Brown were suspended from playing football for one week. That has since been lifted.

Columbus police Chief Elaine Bryant released body camera video Monday showing two Columbus police officers exchanging gunfire with a man who unexpectedly opened fire on them with a shotgun early Saturday, saying "that was an ambush."
Lamar Ivan Blue, 36, who has addresses on the Northeast Side and Far South Side, was critically injured in the exchange but is expected to recover from his injuries, police said.
Officers Garrin Stiltner and Payne Vantilburg, who both have been with the Columbus Division of Police for two years, responded to the 600 block of Kingsford Road on a report of shots fired.
It was the third time in seven hours police had been called to the address for a report of shots fired, Bryant said at a news conference Monday afternoon. Officers had found no evidence of shots being fired during their previous two visits and the 911 caller refused to be contacted, she said.
Footage from body cameras worn by Stiltner and Vantilburg show the officers driving north on Kingsford Road, slowing as they reach an apartment building. A man later identified as Blue can be seen walking down the apartment building's driveway toward the officers.
"What's going on, man?" Stiltner asks Blue as he gets out of the police vehicle. "Did you hear any gunshots or anything like that?"
Blue, his right hand behind his back, answers no.
Stiltner, shining his flashlight on Blue, asks him to take his hand out of his pocket.
Blue suddenly pulls what police say is a shotgun from behind his leg and opens fire, striking the police vehicle. Stiltner and Vantilburg take cover behind the vehicle and at least six shots can be heard in the video.
"Stay covered, stay covered," Stiltner says to Vantilburg.
Officers returned fire and Blue was hit at least once. While wounded he apparently ran from the officers, police said.
Blue was later found in the rear parking lot of an apartment complex across the street. Other responding officers, after determining it was safe to approach him, rendered medical aid to Blue before medics arrived.
"Officers provided life saving first aid until medics arrived,' Bryant said at the news conference. "The officers were sprinting, sometimes at far distances, to their cruisers to get their personal first aid equipment. The officers utilized a chest seal that they bought with their own money to save the life of Mr. Blue."
"For those of us that were there that night, this shotgun blast came perilously close to the officer on the passenger side of this car," said Deputy Chief Greg Bodker.
"This individual (Blue) as you saw, he didn't retreat," Bryant said. "He didn't back up. He didn't try to hide. He didn't go behind a bush and sit the gun down. He walked directly toward the officers. Directly toward them. In my mind, in my opinion, that was an ambush. That's the way I look at it. I feel that that was intentional and he was directing his anger or whatever rage he may have had toward our officers."
There is no indication Blue was behind the 911 calls, the chief said.
The incident early Saturday was the second in nine days that Columbus police officers had been shot at and struck a gunman with return fire, referring to an officer-involved shooting on Nov. 3 when another Columbus police officer with three years with the division shot a gunman in the dumpster area of a South Side restaurant.
The Ohio Bureau of Criminal Investigation is investigating both shootings, as it does with any shooting involving Columbus police officers in which someone is injured or killed. Both Stiltner and Vantilburg are on paid administrative leave.
"That concerns us deeply," Bryant said of the two incidents in which police took gunfire. "It concerns us as a division but it should also concern the community. This cannot be OK. This cannot be OK with anyone that this is occurring."
In addition to body camera footage, police also released a photograph of the vehicle they say Stiltner and Vantilburg responded to the call with. The passenger side door of the door is pockmarked with what looks like buckshot from a shotgun blast.

The Sarasota Police Department is investigating a robbery just after 10 a.m. on Tuesday, November 15, 2022. This robbery call led to an officer-involved shooting that the Florida Department of Law Enforcement (FDLE) is investigating.
An 83-year-old woman, the victim in the robbery, told Sarasota Police officers she finished shopping at the Publix, 501 N. Beneva Road, Sarasota and was pushing her groceries to her car. After unloading the cart, the victim said she was approached by the suspect, who tried to take her purse off her shoulder. The suspect is a 71-year-old man from Sarasota.
The victim struggled with the suspect, said he had something shiny in his hand, and threatened her with it. A witness, an adult man, saw what was happening and tried to help the victim. The suspect started to leave the area, and the witness followed him, videoing him with his phone while calling 911.
The suspect continued walking southbound through the McDonald’s parking lot across from the Homewood Suites, 3470 Fruitville Road, Sarasota. The suspect crossed Fruitville Road, was holding a knife, and was confronted by a Sarasota Police officer in the parking lot of the Homewood Suites between two parked cars.
The officer took his firearm, pointed it at the suspect, and ordered him to drop the knife and get on the ground multiple times. After the officer commanded the suspect to get on the ground and drop the knife, the suspect advanced toward the officer aggressively with a knife in his hand. The suspect was told again to stop, drop the knife and get on the ground. The suspect took several steps toward the officer, and the officer fired his weapon.
The suspect was taken to Sarasota Memorial Hospital and is in stable condition. The uninjured officer will be placed on paid administrative leave, which is standard procedure. The FDLE is the lead law enforcement agency investigating the officer-involved shooting, and the Sarasota Police Department Criminal Investigations Division is the lead law enforcement agency investigating the armed robbery. The suspect is facing charges of Aggravated Assault with a Weapon without Intent to Kill and Armed Robbery.

The Maryland Attorney General's office released officer-worn body camera video from a crash last month that left a teenager from Baltimore dead.
The crash happened just before 6:30 p.m. October 8. According to investigators, officers in northwest Baltimore began following a black Honda CR-V.

The killing of Danquirs Franklin by a Charlotte-Mecklenburg police officer took place on March 25, 2019 when Officer Wende Kerl shot and killed the 27-year-old man.
In the released body-worn camera footage from the confrontation, Franklin appears to have been lowering a gun toward the ground at the time the officer fatally shot him. Kerl was responding to frantic 911 calls about a man with a gun threatening people at a Burger King. The footage shows that Kerl and a male officer instructed Franklin to put the gun down more than 15 times in the roughly 40 seconds before he was shot. The gun is not visible in the video during that timespan. Franklin was shown squatting next to an open car door with another man in the car's passenger seat.
The last command Kerl yells before firing is, “Put it on the ground!” Franklin's right hand slowly appeared as he pulled out a gun by the barrel before he was shot by Kerl twice. Franklin's last words are barely audible, but it appeared that he was saying something to the effect of, "You told me to..." before collapsing.
Franklin's death sparked criticism of the officers' behavior as well as demonstrations following the release of Officer Kerl's bodycam video.
The district attorney's office "could not prove to a unanimous jury beyond a reasonable doubt that Officer Kerl's belief that she faced an imminent threat of death of great bodily harm was unreasonable," wrote District Attorney Spencer B. Merriweather in a letter addressed to the Charlotte-Mecklenburg police chief.
In June 2020, Franklin’s family filed a lawsuit against the city and Officer Kerl. The lawsuit details the events taking place in Franklin’s life leading up to the fatal confrontation. The suit argued he was following the officers’ repeated commands to drop his firearm. Kerl currently works a full-time, non-patrol, investigative role at headquarters.

When a Hillsborough County deputy walked into a Riverview home, she believed someone inside needed medical attention. Instead, she encountered an armed man and was forced to fire her weapon – killing him, according to the sheriff.
The 88-year-old man, identified as Ronald Ehrich, was a retired Tampa police sergeant. The sheriff said he died from his injuries following the Tuesday morning shooting.

Ruby Johnson, a 77-year-old grandmother, was watching TV in her Montbello home when she heard a loudspeaker call from outside – it was Denver Police asking anyone inside to come out with their hands up.
Body camera video shows Johnson, appearing confused and weary, approaching officers outside in a black robe and bonnet with her glasses on her forehead. She looks around, uncertain about what’s going to happen.
“I didn’t want them coming in there shooting,” Johnson said. “I came out, and then they asked me, ‘Do you have a gun on you?’ I said, ‘No, why would I have a gun on me?’”
What happened next on Jan. 4 shattered Johnson’s and her neighbors’ sense of safety, in Montbello – a mostly minority neighborhood in northeast Denver.
Montbello residents who witnessed the SWAT team at Johnson’s home question why police executed the warrant – in a case of a stolen vehicle and guns – that hinged almost entirely on the Apple tracking software Find My iPhone.

Dramatic body camera video footage has been released by the Warren Police Department showing their capture of a suspect who had forced his way into a daycare center last week.
According to a post on Facebook, on Nov. 2, an officer attempted a traffic stop on a male suspect who was wanted for felonious assault. The suspect refused to pull over and a pursuit ensued onto North Park Avenue.
The vehicle pursuit came to a stop when the suspect crashed into a vehicle in the vicinity of North Park Ave. and Washington Street NE. He got out of the care and fled the scene on foot, moving behind the Precious Times Child Care and Preschool.
Officers were close behind the suspect in the foot chase. The body camera video shows one of the officers pulling out a taser and yelling at the suspect to "get your hands up."
The suspect then forced his way through the rear door of the daycare center as an employee tried to close it. As police followed him into center, the suspect raised his hand towards officers, threatening to shoot them. One of the officers tried to tase the suspect, but was not successful.

The Boone County Sheriff’s Office claims Alexis Beaver, 22, fired several shots at her boyfriend, Edwin Romero, 40, Sunday afternoon near the Dollar General store on Verona Mudlick Road. Body camera footage released Thursday by the Douglas County Sheriff's Office shows part of the confrontation and exchange of gunfire between deputies and two car theft suspects Tuesday in the parking garage of a Lone Tree RTD station.
