Today, we know that celiac disease affects multiple organ systems in the body. Because celiac disease can present in a variety of forms, diagnosing celiac disease can be a challenge. Providers frequently will not consider celiac disease if it presents with isolated extra-intestinal symptoms. When the main symptom of celiac disease is depression, this creates one such scenario in which the consideration of celiac disease in the differential diagnosis is often overlooked. Celiac disease is on the rise globally, and the multiple forms of its presentation causes many cases of celiac disease to go unrecognized. This means that people who have celiac disease often discover their illness after complications have already developed. In other cases, the underlying cause of their symptoms may never be discovered. Psychiatric illnesses, such as depression, are prime candidates for these types of missed diagnoses.

Vertigo is often described as dizziness by people who have this sometimes debilitating condition. Most frequently, vertigo comes from the shifting of crystals in the inner ear that is easily relieved by head maneuvers. But vertigo is sometimes more challenging to manage than the well known BPPV (Benign Paroxysmal Positional Vertigo) that is corrected with manipulation of the head. At such times, vertigo can markedly impact the lives of people with this complaint. Diagnosis of these less common types of vertigo, require careful history taking and extensive imaging and electrophysiologic testing. Even with the use of modern diagnostic techniques, it is still sometimes a struggle to get a clear diagnosis of vertigo rapidly. Practitioners who encounter vertigo of this nature are aware that rapid diagnosis can be crucial, as often, effective therapy must be administered in a timely fashion. People who develop these rarer types of vertigo are often unaware of this need for urgency. Central vertigo can pose an emergency and being aware of this can help prevent lost opportunities and ensure better outcomes in people with these types of vertigo. Ozempic is a GLP-1 Agonist that was originally designed to treat type 2 diabetes. It belongs to a group of hormones called incretins that are produced in the bowel. Now, with its sister drug Wegovy, it is sometimes used, albeit in an off-label manner, as a weight loss medication. But can Ozempic become a new treatment for addiction? The treatment of addiction is still not as effective as we would like, so that the addition of new medications, such as members of the GLP-1 agonist class such as Ozempic, would be welcome. Multiple studies are ongoing to evaluate this class of drugs in the treatment of addiction. If these clinical studies prove that GLP-1 agonists, like Ozempic, are effective in treating addictions, this would be a major step forward in the management of addictive disorders.

Psoriasis affects 2-3% of the world’s population. Treatment of psoriasis has evolved over the years, but photo therapy has remained a cornerstone of treatment. As popular a treatment for psoriasis, light therapy is still somewhat of an enigma. Scientists have discovered many potential mechanisms for the healing effect of UV radiation in conditions such as psoriasis. Phototherapy (light therapy) has become more convenient for psoriasis patients, as portable home-based narrow-band UVB machines become available. Psoriasis patients now have the option of treating themselves in the comfort of their homes, and the safety of newer machines means that psoriasis can be treated with relatively low risk of cancer and other complications. Light therapy can be used as a single or combination treatment for moderate-to-severe psoriasis. Psoriasis patients can combine light therapy with the newer biologicals or with systemic medications, giving them an option for the more recalcitrant cases of psoriasis. PHI or Post Herpetic Itch is a serious complication of shingles that rarely do we hear about. That is because, among the complications of shingles, it is the one patients report the least to their providers. This is mainly because it is overshadowed by PHN in most patients. For those people with shingles, however, who develop PHI, it is one of the most challenging diagnoses, because it is difficult to treat. Recently, scientists have been able to decipher much of the etiology of this enigmatic condition, although the exact pathogenesis remains a mystery. With the new understanding of PHI, the treatment of this complication of shingles has received more effective treatments, albeit far from perfect ones. The realization that pathologic itch is a neurological condition has permitted the introduction of targeted therapy, yet this complication of shingles remains a challenge to treat. TTS-Trigeminal Trophic Syndrome directly results from shingles induced pathological itch, and providers and patients alike are fooled into thinking that chronic ulcers cause the often recalcitrant itching that follows bouts of shingles that precipitate this condition. The constant scratching that is precipitated by PHI can have serious consequences. Providers who treat patients with shingles need to be aware of this condition and its peculiar etiology, since many of the usual therapies will not work. When patients with shingles are aware of conditions such as PHI, they can ask questions and learn more about their condition. That way, they can take an active part in their care. Tuberculosis was called “consumption” for a reason. Tuberculosis has terrified humanity since the beginning of time, and today, it continues to cause millions of deaths around the world. The WHO has set goals to diminish if not, eliminate tuberculosis from the world, but so far, their efforts have failed to achieve the desired results. One reason for this setback is the presence of reservoirs of tuberculosis in the global population. Most of these reservoirs are in the form of people with latent tuberculosis infections that constantly reactivate, spreading tuberculosis to previously uninfected persons. Eradicating tuberculosis requires that people with latent tuberculosis receive treatment for their latent disease, which can only be achieved if screening takes place. TB/tuberculosis testing is therefore a pivotal tool in the fight against tuberculosis. In this video I discuss the positive tuberculosis test and all of its implications.

Patients with lupus have reported for years the link between stressful situations and lupus flares. But what is the relation between stress and the etiology of lupus? Graves Disease is an autoimmune disease for which treatment remains less than perfect. Patients with Graves Disease often have to undergo difficult treatment regimens to control their disease. In other cased they are compelled to take medication for the duration of their lives. But recent advances in the understanding of the pathophysiology of Graves Disease have made it possible to follow new therapeutic paths in the quest to develop more effective and convenient modalities of treatment. New biotechnology and biochemical procedures have broadened the scope of possibilities for the development of novel therapeutic interventions in Graves Disease. Many of these new techniques have already been tried in studies in patients with Graves Disease and have shown strongly positive results. As things stand, scientists are on the cusp of a major breakthrough in the treatment of Graves Disease. These new therapies will be more specific and will more precisely address the underlying causes of Graves Disease. These are exciting times for patients with Graves Disease because the new therapeutic interventions are likely to be far more effective and may even effect permanent cures without the inconvenience of surgery or radioactive medications.

Sore throats have been with us since the beginning of time. But what we can easily forget, is that before the advent of the antibiotic era, sore throats were frequent causes of morbidity and mortality. In the past, sore throats led to heart disease and kidney disease, but today, where available, vaccines and antibiotics have decreased the danger this common ailment poses to humanity. A sore throat or pharyngitis has myriad causes, some of which are not treatable with antibiotics alone, and that require urgent surgical intervention. Even today, the complications of a sore throat can occasionally be life threatening. It is up to us to know which types of sore throat pose a danger and which do not. The degree of danger a sore throat poses depends to a great degree on the cause. While the majority of sore throats today are easily dismissed as “just another cold,” some can present medical emergencies.
