An epidermoid cyst (Epidermal Inclusion cyst, Infundibular cyst), is a benign growth commonly found in the skin and typically appears on the face, neck or trunk, but can occur anywhere on the body. Another name used is “sebaceous cyst” but this is actually an antiquated misnomer, and is not a term used by dermatologists. They are also the most common type of cutaneous cysts. Epidermoid cysts result from the reproduction of epidermal cells within a confined space of the dermis. The pasty contents are mostly composed of macerated keratin (wet skin cells), which creates this “cheesy” consistency, and there can be a pungent odor. An epidermoid cyst may have no symptoms and are typically harmless. Usually people seek removal because they don’t like the appearance of these bumps, or the cyst has ruptured or been inflamed or “infected” in the past. Rupture is associated with sudden redness, pain, swelling, and local heat, and can lead to abscess formation. Also, a history of inflammation, often increases scar tissue in the area, makes the cyst more firmly adherent to surrounding skin, and makes it more difficult to remove. Surgical excision is curative, but the complete cyst removal including the entire cyst sac and contents need to be removed to ensure that the cyst won’t reoccur. A Pilar Cyst, aka a trichilemmal cyst, an isthmus-catagen cyst, or a “wen”, looks identical on examination to the more common epidermoid cyst, except that 90% of them occur on the scalp. They tend to be more mobile and firmer than epidermoid cysts, and are derived from a different part of the hair follicle/skin. The internal content tend to be more “homogenous”/ smooth (looks different under the microscope), but it is also a benign growth. It is not uncommon to have multiple pilar cysts on the scalp and to have other family members to have these cysts (tend to run in families). This patient usually sees one of my physician assistants, to treat his acne, but he came in to see me for what we thought was a cyst that was bothering him on his right neck. Well, it didn't end up being a cyst, but something a little less common (but not worrisome): a pilomatricoma. At the end of the video you will see what this growth looks like under the microscope... this is how we make the diagnosis! A Pilomatricoma (aka Calcifying Epithelioma of Malherbe, or pilomatrixoma) is a benign growth derived from hair matrix cells. Usually occurs as a single lesion on the face, neck or upper arms. They cause no pain, but are firm bumps under the skin. The diagnosis is usually made only after looking at the tissue under the microscope. We look for “shadow cells” under the microscope. Calcification and ossification can occur (calcium and bone can develop within) which can make these very firm and hard, much like a pebble or little jagged rock under the skin. These are benign and don’t need to be removed, but if someone requests removal, a surgical excision is needed. Thank you for watching!! A Dilated pore of Winer is essentially a large, solitary open comedone/blackhead. Dead skin cells get trapped and help widen this pore, and plugs up the opening. The expression of this plug squeezes out the macerated, white, soggy keratin/skin cells from the deeper portion of the pore. Once the content of the dilated pore is expressed, this whole process of the dilated pore filling once again with keratin is common. They are completely benign and are usually expressed for cosmetic reasons. A comedone extractor can be used to do this. OK, this is a GREAT pilar cyst excision on the scalp of one of my longstanding patients who is so dear to my heart! She has had this bump on her scalp for many years now, and would love it removed, but wasn't sure what was involved in this process, so she really left it along for many years. She was happy to learn that it is a pretty simple process, and is just a procedure under local anesthesia. I was quite surprised and excited to see that this pilar cyst just popped out whole with just a little pressure and a squeeze! I was happy to do this procedure for her, she was so happy to get it removed, and I'm sure all of you will be thrilled to watch this cyst extraction! Thank you for watching!

This patient was so adorable, because she is down at her family home, visiting from college in another part of the state, and she is accompanied by her mother. I asked her mom and the patient if we could film this procedure and I explained why. Her mom had never heard of Dr Pimple Popper, but she is a popaholic, for sure! Once I showed her @drpimplepopper on instagram, she didn't pay any attention to us or the procedure we were doing on her daughter. Of course, she loves her daughter, but her daughter was very comfortable during our incision and drainage procedure here. Her mom was entranced with my instagram page and she couldn't wait to show her friends. It was just so cute! This patient has an epidermoid cyst behind her left ear, which has recently become inflamed and “infected”. I put “infected” in quotes, because technically this is not an infection: When an epidermoid cyst ruptures, it creates a vigorous foreign body inflammatory response - this skin becomes warm, tender, painful, and swells, simulating an abscess. Incision and Drainage (I & D), will confirm the diagnosis of inflamed cyst, when the cheesy, sometimes odiferous material is evacuated, and this process often leads to rapid resolution of symptoms. These episodes are often misdiagnosed as “infection” of the cyst, but cultures are usually negative and antibiotic treatments is not required. Intralesional steroids can hasten the resolution of symptoms as well. All this being said, we often prescribe antibiotics- it’s hard not to when you see something so red and inflamed and painful.. it seems almost cruel not to! Also, we as dermatologists, will just call the cyst infected, or call it an abscess. It’s more difficult to explain to patients why we would not call an area that looks angry and red and is painful non-infected. However, I felt I needed to clarify all of this in detail here, for medical education purposes! Eruptive Vellus Hair Cysts, EVHCs, appear as multiple, maybe even 100s of 1-4 mm skin colored or hyper pigmented / bluish dome shaped papules on mid chest and upper extremities. These tend to be smaller than steatocystomas and can have a central area of umbilication (almost like a punctum). Steatocystoma may at times have vellus hairs, and EVHCs may have sebaceous glands (oil glands) in their lining, which leads some dermatologists to believe these two conditions may lie along a continuum, that they may be related. Treatment is gentle extraction. There is a high chance of recurrence if a CO2 laser is used to extract them, and topical tazarotene has also been used, but is an inferior treatment. This was a last minute kind of "pop". This is the courier/driver for our dermatopathology slide service. We see him a few minutes on a daily basis at work. Well, a couple weeks ago, he showed me this bump that he had near his elbow, and before he had time to get really nervous about it, we set him up for a quick excision! It was interesting too, and you will see why... So actually, I don't have the exact diagnosis, but I believe that he had a pilomatricoma with a surrounding minor hematoma/blood clot from recent trauma. Unfortunately, when they embedded the tissue, they only embedded the blood clot and not the white "bump" that I squeezed out. He looks great, and we thank him for letting us film this procedure! Keratin (skin protein) and sebum (oil) combine to block the pore. They are often found on the face and trunk. but they can be found anywhere on the body. Blackheads are not clogged with dirt, but it is the exposure to air that causes oxidation turning the internal contents black. They can be extracted using a comedone extractor. I usually use an 11 blade (a blade that comes to a sharp point) and a Shaumberg type comedone extractor. This young lady travelled out of state to come and see me, mainly because she has a mole on her chin that she wants to have removed (you will see this in a future video). She also has numerous small comedones (whiteheads) on her nose and central cheeks and forehead. These comedones are extremely stubborn to extract.. I would call many of them keratin filaments.. just keratin really wedged into her pores/follicles. Since this was her first visit to see me, I recommended she return again after applying RetinA (tretinoin), which is a prescription product, to the area in order to help soften up these comedones so they would be easier to extract in the future. As you can see, her whiteheads were so stubborn! She would have extracted them herself if she had the right tools, but she could really bruise herself trying to remove these. I believe that if she were to start using tretinoin for a few weeks, future extractions will be easier and her skin will look so much smoother. She'll be back in a week and we will see if we can extract more next time! I can't say it enough: I have the most wonderful patients, who are so sweet and so kind to allow me to film them for all you to watch, learn and enjoy!

Luckily, she says she has no other cysts or big blackheads on her body so hopefully this will be the last of them! Stay tuned, I will show you what this area looked like a couple weeks later.
