Quake: Dimension of the Past - E5END: The Year Zero (Nightmare, 100% Secrets)

The final level of Dimension of the Past is basically one giant callback to Shub-Niggurath's Pit, the final level of the original Quake, from the initial fall into the pool with the Rotfish to the Vore and Shambler gauntlet leading to the final boss (except this time you get the Quad Damage at the beginning of the gauntlet rather than at the end making it much easier). However, Shub-Niggurath is nowhere to be found, and her lava island now has a staircase leading down into a new section where you fight a Vore and two Shamblers as the final fight.

This is the end of Dimension of the Past, but not the end of Quake! Scourge of Armagon is coming up next!
