Alec Baldwin shot Halyna Hutchins by mistake during a rehearsal. A live round got into a box of dummy rounds and escaped the scrutiny of the weapons master. The question is, how did a live round get into the box of dummies? Was it put there on purpose by a disgruntled striker? Was it accidentally included in the ammunition provided by the supplier? Did negligent crew members put it there in a hasty tidying up?
