This series documents phalloplasties and the trans men who love them (the phalloplasty-philes). In this installment, a trans man undergoes a Radial Forearm Free-Flap Phalloplasty (RFFF-Phalloplasty), in which most of the forearm is removed and rolled up to create a "tube within a tube" construction. Their term, not mine. I would have called it "that gift from the nature shop at the mall that was shimmery and really hard to hold onto - what was the name?" construction.

My phalloplasty technique, called the Exulandick, involves taking a 3D printed silicone mesh with multiple internal chambers designed to mimic the chambers and tissue tensility of the actual penis, with none of the harsh side effects of traditional RFFF Phalloplasty OR any of its less shameless subsidiaries. A partial thickness graft is then taken from the thigh and this is used to dover the Exulandick and provide it with that natural look and feel. The patient is woken up mid-surgery to confirm they like the size, and if not, fat is grafted from the face, right under the eyes, to between the Exulandick and the thigh skin, until the patient consents to the size. If the patient changes their mind later, after anesthesia wears off, they can pay for a degirthing procedure at that time.

For the transman that wants to have Exulandicks of various shapes and sizes, but does not want to sacrifice that natural skin feel, we recommend a marsupioplasty, in which the surgeon creates a semipermanent Ventral-Caudal Diverticulum, or "kangaroo pouch," into which different Exulandicks can be swapped in and out. As long as you clean it regularly with the pouchwash we recommend, Exul de Toilette, it probably won't get too infected that often. If it does, oh well.
