Who was creating this money?

In the second half of the 1950s US dollar-balances (money) began to appear 'out of thin air' in London, Europe, Japan, etc. Who was creating this money? We're not sure. But WE ARE SURE where it WAS NOT coming from: the US Federal Reserve.
