Request Form. (Top Left)
Symptoms > Vaccine > VAERS ID

(Skip down to 3, under 'vaccine products')
☑️ COVID19 Vaccine
4, select all locations
5, death
Skip down to 7, scroll to bottom of years, click 2021 only. For steps 7 thru 11.
