To Whom It May Concern,
Regarding: The requirement of Covid vaccination, but not limited to, ALL vaccinations, titers, mandates and unconstitutional violations of personal self-liberties, and regarding faith, and the knowledge of being fully informed of consent.

I. I would like to clearly state: I am requesting, by my personal faith, the liberty to decline the Covid vaccination, including, but not limited to (such as: flu, MMR, pertussis, hepatitis), ALL vaccinations, titers, mandated injections and the similarly classified ‘requirements.’

I take my faith very seriously, wholly based upon the inspired Word of God as found in the Bible (2 Tim. 3:16, 17) and applied by personal conviction and faith. I seek to honor my faith and obey my God (Js. 4:17) by living out all such realities. I am convinced I am to keep my entire being ‘blameless and pure’ to the best of my abilities and under the sight of my Lord (1 Thess. 5:22, 23). In living this faith, I seek all areas of my life (1 Cor. 6:19, 20) to conform and apply biblical precepts and teachings regarding my spiritual and physical well-being, both in the physical present and the eternal future.

Should the formerly mentioned not meet your judgment and interpretation of my freedoms, liberties, convictions and personal life decisions, and should my rightful freedom be stripped, forcing me into subjection by tyrannical means of physical violation and intrusion into my physical body, then I reserve the right to the sections below in order to make a fully, truthfully, honestly, and rightfully sound and informed decision as to whether I should live out my faith or sacrifice my career.

II. As stated above, and adding to this, as necessary to make a decision with a good and clear conscience, fully disclosed with FULL informed consent, I would like to ask to be provided with a few answers to the questions below, as is my HIPPA and patient care rights, to ask any, all and every question that I may have regarding the personal care and involvement of my personal, private and intimate health.

A. Can you please advise me, in written form, of the approved legal status of any vaccine, especially as it pertains to the mandated/required vaccinations (i.e. Covid, flu, MMR, pertussis and all future and etc.) and if it is classified as experimental?
B. Can you please provide details and assurances, in written form, that the vaccine(s) has been fully, independently and rigorously tested against: 1.) control groups, and 2.) the subsequent outcomes of those tests?
C. Can you please advise me, in written form, of the full list of contents of the vaccine I am to receive, with factory labels and inserts attached, and if any are toxic to the body? If any toxins are contained within this vaccine, can you please provide me with the information as to the harms, damages, possibilities and their history in being used inside the human body?
D. Can you please fully advise me, in written form, of all adverse reactions associated with this vaccine, and possible reactions, both historically and scientifically, since the introduction and initiation of this vaccine?
E. Can you please confirm, and provide me the information in written form, that the vaccine you are mandating/requiring, and advocating, is NOT experimental mRNA gene altering therapy?
F. Can you please advise me, in written form, and signed by an official representative of your company, confirming that I will not be under any duress, undo stress, and suffering, from yourselves as my employers, in compliance with the Nuremberg Code?
G. Once I have received answers to the above health questions in written form, and I have had adequate time to read, fully comprehend and understand my informed consent, I would then need to ask a few more questions so to be fully educated in making this decision.

A. Will you confirm, in writing and signed by your company representative, that I will receive no personal self-harm resulting from your mandated vaccine injection?
B. In the event of vaccination injury: can you please advise me, in written form, and signed by an official representative of your company, of the likely risk of serious vaccination injury, and your company’s (my employer) responsibilities should I become unable to communicate, or succumbing to fatality, after the injection of your mandated vaccine? Can you please provide the process for your responsibilities regarding all of my medical expenses, burial needs, and fully providing for my family’s living expenses and personal needs?
III. Please be advised, that my inalienable rights are reserved.
IV. Following acceptance of my request for to be fully disclosed and informed of my decision to consent, and if I should decide to receive the mandated vaccine, this must be signed by a fully qualified doctor and company leadership representative, who will assume full legal and financial responsibility for any injuries, regardless the severity, to myself, and/or from any interactions by any personnel regarding direct, or indirect, injuries by said persons.
V. Be advised, in the event that I should decline the mandated vaccination, I am in no way seeking to resign my position with your employment.
