Phone Directory Project [Part 19] - Creating Insert Phone Record Controls

So for the most part, we've completed the phone directory in terms of making it spit out the phone directory data from our database and making it look pretty. Now, we'll be adding a new feature that will allow us to add new phone records! Let us know in the comments if you have any other idea's that you think we should code!
