How to submit an incident on the Service Portal for ServiceNow [Paris]

Keep in mind this is for those that are using out-of-the-box functionality. Some Service Portals are fairly customized and yours may look different. However, almost all of them have some type of Get Help or Submit Incident type of header on the main page.
