In this video, I’m going to be covering the first step in providing SMART Emergency Medicine - Making sure the scene is safe.

I came up with SMART emergency medicine, which stands for Scene safe, Massive bleeding, Airway, Reassess, and then Transport.

SMART is a simple, effective system to respond to any emergency medical situation before medical personal arrive, or the victim can be brought to a hospital.

If you haven’t watched the overview video, make sure you watch that before continuing so you can see how all these pieces fit together to provide a framework for protecting yourself and those you love … or, even those you just don’t want to bleed to death.

If you come up to an emergency medical situation, you may or may not know what happened to cause the injuries, but obviously, you don't want yourself or others to become casualties as well, so you need to check and make sure the scene is safe.

Especially in a gunfight or other shooting situations. We have a saying in the SEAL Teams - The best medicine is fire superiority!

Sometimes the best medicine that you can give is stopping the person who is putting holes in either you or the ones you love; so you may need to return fire or get to a safe place before you can even start treating the patient.

Whatever the situation, if you didn’t see it happen, take the time to look around and see what might have caused the injuries.

Look for other people injured, look and listen for gunfire, downed power lines, fallen items like a ladder or rocks, or rabid guinea pigs - anything that could hurt you or others before you can help the patient.

I want to thank the sponsor of this video, LA Police Gear.

I’ve been teaching people how to safely and effectively use firearms for nearly 30-years now and I’ve found most people buy the equipment - a gun or a rifle - and think they’re safe - ‘they don’t need any training’ - but with emergency medicine, it’s just the opposite. People that get trained often don’t follow through and buy the first aid kit they need.

Luckily, LA Police Gear and North American Rescue have teamed up to design some amazing first aid kits to make it easy and affordable for everyone to have all the essentials you need in a compact pouch you can bring with you anywhere.

The one I use is the: LA Police Gear - North American Rescue Bleeding Control Enhanced First Aid Kit that comes with a CAT (or Combat Application Tourniquet) and a QuikClot EMS Dressing. Combine those critical life-saving tools with everything else in this full-featured medical kit, all at an insanely low price, and you've got a no-brainer first aid kit to give you the peace of mind that you’ve got all the tools you need to put this medical training to use.

And speaking of training, I highly recommend you purchase at least two of these kits so you can practice with the equipment before you’re trying to figure out how to open a package or apply a bandage under stress.

I keep one of these kits with me in my “man bag” that I take with me everywhere I go, and I also keep an extra one in the “man van” in case I need it for a massive pile-up or other major disasters.

now when you show up at the scene with your new LA Police Gear First Aid Kit, and you’ve practiced using all the supplies in it - you STILL need to make sure the scene is safe.

And one thing I learned in 30 years as a combat medic, in any emergency situation, assume no one around you knows what to do, and assume - that they will freeze.

I’ve seen it too many times to count. It’s your job to take control. Tell someone to call 911 or to go get help. Don’t make a general request to the group.

Make eye contact with someone (..see if there is anyone awake insidE), and tell them exactly what to do.

“You in the blue shirt! Call 911!” And make sure they acknowledge you.

Okay, make sure you practice the lesson you just learned. In this case, situational awareness. Do walkthroughs with your family and friends because just watching this video won’t save anyone’s life.

In the next video, I’ll be teaching you the M in SMART Medicine - Massive Bleeding.

HINT: It would be good to have the medical supplies I’m going to be talking about in hand to practice along with me.
