A $290,000 Australian government report by Deloitte has been exposed for containing fabricated quotes, false academic citations, and references to reports that don’t exist. The firm has now agreed to partially refund the government after admitting that generative AI (Azure OpenAI GPT-4o) was used in drafting the document.

This is more than a corporate blunder. It’s a reckoning. When consulting firms, governments, and institutions rely on AI without rigorous verification, lives, policies, and public faith are all on the line. The errors in this Deloitte report weren’t minor footnotes, they involved misquoting a federal court judgment and inventing academic sources.

If we let reports like this slide, we invite systemic breakdowns. What about welfare decisions, legal judgments, public policy, or individual welfare cases that rest on “expert” reports? Once trust is broken, it’s difficult to repair, and that loss of confidence can mean real financial and human harm.
