Demons and Angell

Nurses Dave, Nicole, and Ashley have an ongoing grievance against the state of “evidence-based practice” in healthcare, often diving into controversies and lawsuits that show how our systems are hijacked by bad actors.

Many healthcare professionals, academics, and scientists have only recently been waking up to how dire the situation really is - one silver lining of the Covid fiasco. Others have been sounding the alarm for decades; one is Marcia Angell, who was once Editor-in-Chief of the prestigious New England Journal of Medicine.

After leaving her post at the NEJM in 2000, Angell began an advocacy tour calling attention to the state of fraud in clinical research. In her 2004 book The Truth About the Drug Companies: How They Deceive Us and What to Do About It, she stated:

It is simply no longer possible to believe much of the clinical research that is published, or to rely on the judgment of trusted physicians or authoritative medical guidelines.

Angell made eight suggestions for saving clinical research and evidence-based medicine. How have things changed in the past few decades? The nurses are able to relate many of their prior episodes directly to the issues called out by Angell years ago.

Moving forward in time, the nurses land on recent revelations regarding the explosion of what is essentially organized crime in clinical research: paper mills, an industry that exploits the publish-or-perish state of academia to flood the system with junk science, garbage, and harmful fraud.

As if those problems aren’t enough, the nurses also discuss the bad actors who are gaming PubPeer, a crowdsourced peer-review system, in order to take down legitimate clinical research - for a price. In a sign of the absurd times we are living through, this drama is unfolding on X in a scandal tagged PubPeerGate, frontlined by an account that distinguished itself during Covid called “Jikkyleaks.” Essentially, there are mercenaries who will gladly destroy those slivers of truth that do make their way to the surface that might threaten industry profits or power centers.
