Guide:
22:44 - Telephone encounter doctors note- Emily B at 7/1/25 at 11:25 AM "PT called stating he no longer wants any help from us, he has thrown away all of his medications, nothing is helping him [...] Nicole got fired [...] he said notes from the ER notes are not true."
24:43 - 8/14/24 discharge summary note is included.
Note visible: "He also feels like he was mistreated here as well specifically as it pertains to being asked to do things that he claims he cannot do on his own, such as to wipe himself after having a bowel movement and showering on his own. He was perseverating on these issues quite quite significantly, and I could not really even redirect him for more than a minute or so before he would return to these topics. Patient left AMA Yet again today. [...] Spoke with Liberty Healthcare corporation- That company does not think he needs a guardian and thinks he is capable of making decisions [...] APS thinks he does not need a guardian. Has 2 medical eval stating he does not have capacity and needs a guardian."
"[...] they attempted to get him formal evaluation by psych or neurology but the patient was unwilling to undergo the evaluation. States they have a court date later this month to determine the next steps. Needs neuropsych consult for full evaluation. Ran away from group home today."
26:32- New medical record from UPMC Hamot on 7/1/2025. Andrew had a "behavioral outburst" because his caregivers left early which was "concerning to staff" and was brought to the ER. Andrew was demanding to go to an new assisted living facility. "During his emotional outburst patient tore up and throughout [sic] his diapers, pill packs, and poured lotion on the floor*.
26:43- A long list of medications that Andrew is taking. The page immediately after the dictation notes Andrew's lip twitching and states, "possible dystonic reaction in the setting of multiple psychiatric medications." This hints that if (emphasis on "if") these lip twitches are genuine, they are the result of potentially being overmedicated. The document describes Andrew's speech as being a "flight of ideas with tangential thought process."
27:50 - In some of the visible documents, Andrew he lies about only having gotten to 8th grade in school. (He stopped attending high school just before graduating after purchasing a counterfeit degree online.) There' s conflicting self-reporting about Andrew having a brain injury or not (one page says yes, another page says no in the same document). When asked who he would rather live with, Andrew chooses "child" among other options, even though he has no children of his own.

Lastly, two Crisis Services calls:
The first call was made at approximately 11 AM July 5.
41:55 - Crisis Services operator Christian answers; Andrew uses an intense "baby voice."
43:10 - Andrew is still attempting to get himself 302'd (involuntarily taken to the hospital). Based on what he's saying, it sounds like Andrew had made Crisis Services calls the previous night (July 4) as well because "his caregiver left early."
47:54 - Andrew attempts to manipulate Chrisitan with his "I'll take my meds as and if I take them wrong, I'll just get sick" routine, implying he'll put himself in danger if Christian won't 302 him.
49:10 (approx.)- Andrew's put on hold.
54:16 - Call resumes. Andrew says since there's no safety concerns, he's going to go try to get someone to help him / go to the store to get milk (hard to understand exactly, he's babbling).
56:10 - Thinking he's outsmarted the Christian, Andrew hangs up.
56:53 - Another Crisis Services call, date and time uncertain, likely not long after the first. Christian again. Andrew uses the Avaz app to talk for about 2 minutes straight. He then accuses Christian discriminating against him, (and turning the hospitals against him?), then says he hopes Christian is happy that he almost got hurt. Andrew then hangs up.
