Maintenance and training blamed in boat fire

Poor maintenance, untrained staff and U.S. Coast Guard's failure to require repairs before issuing a passing grade on inspections are listed among factors that led to a January casino boat fire in Port Richey, FL which resulted in one death and more than a dozen injuries.
