In this conversation with Dr Paul Hobday, we get into a passionate and at times challenging exchange about the state of healthcare in the UK. Paul, a retired GP and vocal defender of the NHS, believes the solution lies in returning to its founding principles and resisting privatisation. But I pushed back, hard. Because for me, the issue isn't as simple as NHS good, private bad. The real cancer sits deeper: the centralised capture of medicine by Big Pharma, from the journals to the curriculum to the regulatory bodies that now control how doctors think and behave.

We talked about how protocol-driven care has reduced doctors to technicians and created a system where clinical curiosity and personalised treatment have been replaced by tick-box compliance. I argued that the NHS, far from being truly public, is now a facade, outsourced to private companies, harvesting public data for Big Tech firms like Palantir, and effectively creating a captured market. It's not a sanctuary of care, it's a global testing ground.

I don’t believe the answer lies in restoring some mythical golden age of the NHS. It’s about building decentralised systems, creating genuine competition, empowering community-based hospitals, and freeing clinicians from the institutional straitjackets of the GMC and other corruptible central authorities. This wasn’t a cosy agreement, this was a necessary clash of views, and one that I hope opens the door for serious rethinking about where true healthcare reform must come from.

That said, I think by the end of the conversation, Paul and I agreed that we had more in common than not.
