Accident Summary:
Flight 228 had departed from Windhoek on the second leg of its journey to London. Shortly after takeoff during the night and in total darkness, the aircraft crashed approximately 5 km (3.1 miles) northeast of Windhoek-Strijdom Airport.
A massive post-impact fire resulted from the plane's high speed and shallow descent angle when it crashed into the ground. The impact and resulting fire claimed the lives of 123 people, including all 7 crew members in the cockpit. Only 5 passengers seated in the rear of the aircraft survived.
the Investigation:
The investigation was conducted by South African authorities, with assistance from the U.S. National Transportation Safety Board (NTSB) and Boeing.
Primary Cause:
The aircraft's controlled flight into terrain (CFIT) occurred as a result of the aircraft's premature descent following takeoff, primarily as a result of pilot error and poor management of the departure procedure.
