To get access to it do the following!

1: Just before the NIGHTMARE portal hit the metal plate on the wall.
2: Enter the Nightmare Portal
3: Enter Episode 2
4: Complete all the maps before Crypt of Decay!
5: Go to the location marked as [Quad Damage]
6: Follow the ledge on the side of the wall (near the Quad Damage)
7: Jump into the small pond of water.
8: Achievement Unlocked :)
