The Document, Below, is for you to copy and paste and exchange the word “Amazon” for which ever entity (business…ie..website) you are contacting, to lodge a CLAIM (below) concerning the fraud against Judy. Take out the quotation marks, I put them here so that you don’t copy the whole description:

“Concerning this fraudulent business AKA “drjudyamikovits.com” operating on this Amazon Sellers page; I have been made aware by the woman named Judy A, Mikovitz, that this aforementioned business is committing fraud against her by use of her name (as their aforementioned website “business name”) and by selling this woman’s’ books on Amazon and doing so thru a link on their website that directly sends the public at large to this Amazon Sellers website.
This Amazon Sellers Page is deceiving your customer base into the belief that they are buying from the real woman named Judy A, Mikovitz. This fake, false and fraudulent business illegally and unlawfully represents itself to the public at large, and by and thru the allowance granted to them by Amazon, to use this Amazon’s Seller Website Services, are thereby being aided and abetted by Amazon, to continue committing their fraud against the woman named Judy A, Mikovitz.
I hereby require that Amazon close, end, delete, terminate and forbid this ‘business’ AKA, drjudyamikovits.com, from use of Amazon websites and or affiliates in control, ownership, possession of and or property of Amazon. Honorably, I Thank You”

To Judy, Please Make The Above Changes To Your Plea For The Public To Assist You In Ending The Fraudulent Websites That Are Using Your Name And Or Selling Your Books!
What you have on your site holds no legal nor lawful “claim” and is void of any consequence, whatsoever, so these ‘pleas’ and or “complaints” to any of these sites, will be ignored.
You should try to have a statement that will make this (these) entities take these frauds against you seriously. So ask people to write this type of notice with a “Requirement” to remove these sites and a notice of the potential “Liability” of their continued allowance for their site(s) to be used to commit the fraud by these fraudsters!

Making a “Complaint” is alike to a woman being raped and while she’s being raped she decides to hold up a “Protest” sign with the writing of “Please Stop Raping Me”; an entirely useless endeavor. Whereas, “A Claim”, is in direct opposition to a weak, meaningless “Complaint or Protest” Because it holds and remits legal ramifications!
You are notifying the entity (AKA “Business”, in this example “Amazon”) that they are “Aiding and Abetting” (notifying this entity that they are placing themselves in a position of liability) in the actions of “Fraud”, “Against a Woman” and you are telling their “Legal Department” (by entering this verbiage into their “Customer Service” website, designated by this entity, as a place to lodge your concern about the business using their site) that you “Require”(the legal term for DEMAND) that they take action within to “End Their Allowance” of this fraud against this “woman” to continue.
