Note: Every 4th Entrance into the Fishin Pond will spawn 1 Hylian Loach.
Which will swim under the water leaves.

So if you collected everything perfectly (10 pounds fish) + Golden Scale then your entrance count is 2
There for by entering 1 time and quit instantly and then exit to re-enter the Hylian Loach will be spawned.

Are you not aware of how many times you went fishing?
Check under the water lilies before exiting again.
