No matter the order of dialogue with Patches, he will always betray you by kicking you into the pit (if you get that far). If you kill him before he betrays you, he will basically ask why:

"What did I do? What ... did ... I ... do?!"

I included this as part of the "Killing friendly NPCs" series because I killed him before his betrayal rather than after.
