KCD2 - How to Brew Perfect / Exceptional / Henry's Potions

Are you having trouble with the alchemy/potion-brewing system in KCD2?

I did. Totally ticked me off. It is VERY different from the original game. I honestly think it's better, but the tutorial is terrible and - even if you go through it with a fine-tooth comb - you'll still have to experiment to get it to work.

So I figured out how to do it perfectly and I decided to - very lazily - run through the process for you guys so you don't have to suffer through trial-and-error like I did.
