Pediatricians that vaccinate children as far as I'm aware of do not provide this information to their patients before administering vaccines. There's something under federal law they're supposed to give vaccine information sheet. The 1986 act required the CDC create short little thing called vaccine information sheet that has certain disclosures and under federal law pediatricians are supposed to provide that before they administer vaccines. I know that it's common for them not even to give that minimal disclosure.
