Pagamutan ng Dasmariñas and SWA Office Requirements for medical assistance

After Mark's hospitalization and funeral, our family is really short on funds for another bout with hospital bills. We prayed, asking God to have mercy and provide our needs as He has promised (Philippians 4:19). We went to Pagamutan ng Dasmariñas, a public hospital to avail of lower medical bills' cost. Though the cost was lower, we still can't afford them so we went to the Social Welfare Assistance Office (an extension of DSWD through the City Mayor's Office) for medical bills financial assistance.

I listed in this video the requirements. Kindly screenshot them and check with the SWA office for your medical bills' deductions. This process only applies in Pagamutan ng Dasmariñas. Take note the requirements may change anytime.
