For one campaign I was a player in, it was set in the first level of the Nine Hells. The lore was that we died and since we were evil, we went to the hells. Yurnec was a plague-spreading paladin in his life who was defeated and killed for his evil deeds. There we began making connections and eventually, Yurnec, became a servant to the ruler of the First Level of the Hells. He accrued a large amount of power and influence while in service but eventually, he was powerful enough to confront his master. In the end, Yurnec usurped the First Level of the Hells and became its master.
