My experience in what caused overcharging or ballooned cells; and once, a blown fuse:

Dropped battery.
AC ripple on Daymak maxi scooter.
Damaged BMS.
Mistakenly plugging cord into Motorino pack instead of the charger.
Motorino BMS without active cell balancing functionality.
Battery age.
Charging when temperatures or moisture forbid.
Charger without temperature compensation functionality.
Vibration that made charger output become erratic.
Vibration breaking connections or circuit board traces.
AC voltage or spikes exceeding charger's capability.
Cells without Hi-V terminating circuitry, as used in almost all packs to obtain the High-C rate desired by consumers.
Wrong charger, or setting, or charge profile, for the cell chemistry being charged.
Rainwater or moisture in pack.
Wiring cells with different Internal Resistance together.
Moisture ingress in wiring.
Missing BMS in any multi-cell series pack.
Invisible corrosion on connections.
