DS-12: Learn to clear failure-to-fire malfunctions

There are three common ways for handguns to malfunction:
— The first is failure-to-fire. This is when the trigger went click but the gun did not fire normally.
— The second is failure-to-eject. In this case, an empty casing is stuck in the ejection port.
— The last is failure-to-extract, when an empty casing is stuck in the chamber, and a live round tries to feed in behind it.
