Ryōma's world is shaken as he uncovers the unsettling demise of Sixth Division Captain Inoue Genzaburō, a revelation that sends ripples of unrest through the core of the Shinsengumi. In the midst of this turmoil, accusations directed at Ryōma for the murder of Inoue escalate, with Okita emerging as the accuser. However, Kondō steps in, peeling back the layers of deception to unveil the startling truth.

The facade shatters as it is revealed that Inoue, concealing his true identity as Serizawa Kamo, had been coerced into an assassination attempt against Tōyō by Ryōma's impersonator. Further intricacies emerge, as the real Okita, Inoue, and Nagakura had previously targeted exiled Mito comrades Hirayama Gorō and Hirama Jūsuke. In a fateful turn of events, Serizawa had taken their lives in self-defense. Kondō and Hijikata opt for secrecy, orchestrating Serizawa, Hirayama, and Hirama to adopt the identities of Inoue, Okita, and Nagakura. This charade extends to the transmission of their martial prowess the Tennen Rishin style to other Shinsengumi leaders.

Kondō further discloses the shocking revelation that the enigmatic imposter Ryōma had sought refuge within the walls of the Teradaya Inn. A climactic confrontation unfolds at this very location, as Ryōma and Okita stand face to face with the imposter. Amidst the clash, Ryōma's keen eye discerns a telltale scar on the imposter's back, reminiscent of Takechi's mark. In a twist of fate, the imposter manages to slip through their grasp, leaving behind a trail of uncertainty. The tense scene is abruptly interrupted as the Mimawarigumi make their entrance, wielding authority to wrest control of the case from the Shinsengumi, thereby thrusting the narrative into new and uncharted territory.

Like a Dragon: Ishin! (龍が如く 維新! 極, Ryū Ga Gotoku Ishin! Kiwami, lit. "Like a Dragon: Restoration! Extreme") is a remake of the 2014 game Ryu Ga Gotoku Ishin!, which was a Japanese-exclusive action role-playing game historical thriller spinoff of the Yakuza/Like a Dragon series released on the PlayStation 3 and PlayStation 4.

This remake was developed by Ryu Ga Gotoku Studio and released worldwide on the PlayStation 4, PlayStation 5, Xbox One, Xbox Series X and Series S, and Microsoft Windows on February 21, 2023, with owners of the Digital Deluxe edition being granted access on February 17, 2023.
