Samoa measles outbreak - unanswered questions

People often talk about measles outbreak in Samoa in late 2019. A lesser known fact is that 2 babies in Samoa had died following administration of the MMR vaccine in 2018, following which the vaccination campaign was temporarily suspended. Oddly enough, instead of questioning the vaccine, 2 nurses were charged and imprisoned for 5 years for "incorrect administration". Samoa restarted a measles vaccination campaign.

Measles vaccination restarted in April 2019 and shortly after there was a measles outbreak.

Unanswered questions
1. Why was the vaccine absolved & nurses blamed?
2. Why did the outbreak occur only AFTER the vaccination campaign restarted?
