The main reason you are struggling to even change your habits is.... because of a habit (big surprise *eye roll*). The habit that we all hate hearing about: health (insert another eye roll). The reality is that we cannot escape it. Our physical health affects our emotional state, which in turn causes us to have either a good or a bad mental state depending on how our physical health is. Even if you are in bad health, if you are practicing healthy HABITS where you practice temperance and mindfully choosing - as opposed to emotionally choosing - you're overall mental and emotional health will be in a good state.
