This poem appears in the biography that Ronald Knox wrote for Lieutenant Commander Patrick Shaw-Stewart:

The poem isn't titled, but was found written in Patrick's copy of 'The Shropshire Lad'.

It is #ANZAC day here in Australia, when our inability to learn the lessons of War is pondered.

Patrick was killed in action (by a shell) at the age of 29. While Patrick was a Lieutenant Commander in the Royal Navy when he died, he called himself a 'Major' in letters, possibly because he spent time working with the Army.
