Dans le cadre d'une étude de cas,une équipe de chercheurs détaille le cas d'un homme de quarante cinq ans dont le partenaire avait inséré une longue paille dans son urètre relié à une bombe de mousse isolante pour tenter d'atténuer sa dysfonction érectile, avant d'appuyer par inadvertance sur la gâchette. L'isolant a donc traversé l'urètre sur toute la longueur avant de finalement remplir la vessie. Une chirurgie en deux temps La personne affectée s'est présentée aux urgences avec des difficultés à uriner et un peu de sang dans l'urine environ trois semaines plus tard. La tomodensitométrie (TDM) de l'abdomen et du bassin a révélé le corps étranger. La personne affectée a ensuite été emmenée au bloc opératoire.« La mousse de pulvérisation a d'abord été retirée de la vessie et de l'urètre prostatique par une cystotomie« , écrivent les auteurs. « Une tentative a été faite pour retirer la mousse pulvérisée dans l'urètre par endoscopie de manière antérograde de la cystotomie à l'aide d'un panier, mais la personne affectée souffrait d'un rétrécissement urétral et les événements étroits de son urètre ont ancré la mousse pulvérisée en place« . La personne affectée ayant déjà inséré divers objets dans son urètre avant à des fins de « gratification sexuelle », cette complication pourrait être due à ces objets.Lors d'une opération distincte (urétrostomie périnéale), les médecins sont finalement entrés dans l'urètre par un trou percé dans son périnée. L'équipe à alors pu récupérer la mousse restante. Cependant, d'autres opérations seront nécessaires dans le mais de « réparer » complètement l'urètre du patient. Dans la partie « dialogue » de leur étude de cas, les médecins rapportent que les personnes habituées à s'insérer des corps étrangers dans le tractus génito-urinaire souffrent souvent de troubles mentaux. « Malheureusement, de nombreux souffrants sont des récidivistes et, par conséquent, une évaluation psychiatrique pour prévenir les blessures récurrentes doit être diffusée« , peut-on lire. Concernant le cas de ce patient, ancien sans-abri, il n'a pas été référé en psychiatrie en l'absence d'épisodes répétés depuis son opération.
