I use the Tmobile IP relay service for calls for medical reasons primarily due to anxiety, suicidal thoughts, and depression. I can speak if absolutely necessary but I do not prefer to. I prefer nonverbal communication (emailing, writing, texting, IP relay etc). I do not need a sign language interpreter. I do not have issues with hearing. I can receive calls and texts at this number but all calls are redirected to relay.

Under the accommodation guidelines set out by the ADA, I am requesting the ability to conduct business with you by email or IP relay because I am a disabled individual with mental health and memory issues.
