Low Carb Down Under podcast

1996 paper:
-lesioning the sympathetic trunks at
the L2 & L3 levels
"Patients who had failed to improve from
lumbar laminectomy operation;
had localized anesthetic injections at
L2, L3, L4;
then 15 lumbar sympathectomies
were performed
Results: 11 patients
-were satisfactory after 6 months!
-4 unsatisfactory: poor results from earlier local injections

Conclusion:
lumbar sympathectomies can be great
for those with severe low back pain
-localized injections can help identify
patients who would benefit

CASE STUDY: 69 year old female
[L3 to S1 fusion]
Operation cured left leg pain, but
increased right leg pain
-did not respond to injections
Radiofrequency lesions at L2 & L3 results:
-pain gone and 3 meds ceased

Closeup of nerve...red small nerves
insert into periphery of larger nerve
[nervi nervorum--supply sensation to
nerve itself]
-nervi nervorum signals use sympathetic
nerve pathways...visceral
"Thus poorly localized: so produce
achy type pain"
-irritation of a nerve, even without
compromising its function,
CAN CAUSE PAIN in a predictable pattern

Example: pain around the umbilicus...
prior to apendicitis, or
-left arm pain prior to heart attack
"Irritation of nervi nervorum can be:
-mechanical, from direct pressure
or
-chemical, from inflammation
