Before collecting the last pages of the manual, go and defeat the final boss. If you do complete the manual first, this achievement will not be available during the current playthrough. However, if you fight the boss and get to the credits before completing the manual, you will unlock the achievement and have the option to Retry. Retry will let you jump back into the game and complete the manual.

Before engaging The Heir, make sure you have your Fire Wand and Ice Dagger equipped so you can do the ranged ice attack (pressing both at the same time). Fight the boss near the edges of the arena when able. When you need to heal, quickly roll away from the boss to give you enough space to heal before the boss closes the gap.

At the start, use the Fire Wand and Ice Dagger combo to freeze the boss and attack with your sword. Next, focus on dodging attacks and attacking while she is open. If you land enough consecutive attacks, the boss will drop to her knees and give you many free hits. Then, in phase 2 the boss will use attacks that drain your health. Be extra vigilant in not standing in the pink/black sludge.
