Kills (418/426), items (120/121), and secrets (100%). This level is "Black Tower" from Stavi's WAD, Bloody Vanilla Master Levels. This map certainly brings back the memories as I've played this in Williams Final Doom Homage. Some arachnotrons failed to teleport, what the hell?
